Old man with SOB.u did the following xray (showing Peffusion )

*Talk about the possibilities
The guy was very friendly ,he told me he had SoB for last ¾
months,gradually increasing ,previously could walk long now not even being
able to do his own shopping,has cough,no hemoptysis,no fever,chest
pain,chills,no travel history ,contact history,long term smoking history was
there..

I talked about all the d/ds of PE esp focused on lung CA with lots of reassurence.


Feedback:failed ! with 2 in history and diagnosis and d/d!
Have absolutely no idea why!

S2.Primi with Placenta previa (Grade 3/4 ),diagnosed on USG 1st time at 36 week
probably,in a GP clinic 250 km away .no history of bleeding before.

Task * explain the result

Explained with pictures of grading
Ask a couple of question regarding the risk factors,nothing was there
Send to Tertiary hospital now for admission and talked about CS possibilites

S3.Women with PV discharge,typical case of vaginal candiasis.

Asked all qs of discharges and 5Ps
Followed typical steps of examination in female (Got 6 in examination sequence
so best to cover it like GE,then systemic,then Private part with
inspection,Perspeculum and PV )

S4.Child with irritability for 5/6 days,and with lump on the neck.

child getting sick for a week,noticed a lump day before yesterday,
Have fever morederate ,no other positive systemic signs,on examination
tympanic mem was dull,lymphn node was very tender and fluctuant baby was
very irritable.I dxed as infective lymphadenitiss and gv a course of Antibiotic and
send home with red flags.

S5.NVD versus CS,Wanted to do CS for quick and easy way,role player was very
rude,wasn’t letting me to talk,

Everything was mentioned in the qs so wasn’t much to ask in
history,mainly focused in counseling.there is one detailed recall for thos
q,please follow that

S6.Thyroid exam ,girl with lethargy ,TFT was given

*State dignosis and further mx

Did PE,findings weren’t not clear and very confusing
I used to had the impression that when u will do examination the examiner will
gv u finding,which is not right,at least not with me.In no PEs,none of the
examiner esp this one uttered a single word.So I said Hyperthyroid based o TFT
even though the c/C was lethargy and feeling sleepy :/

S7.Patient with unsteady gait,
history of long alcholism,

*State differentials

Did gait ,ITPRC ,with only co ordination signs were positive,Dxed as cerebellar
ataxia

S8.Headache for few month probably,cannot remember detail stem,Jaw
claudication was there most probably

It was a total mess.i had no idea what to do,so asked the pt a few qs,and did
inspection,palpation like facial trauma case and did all Cranial nerve examination
till 7.No snelles chart ,no red pin,no tuning fork so skipped those.and didn’t do
fundoscopy otoscopy bcz examiner said ‘I cant dim the light and there is no
dialator,do u think u could do it?’
I told patient all the d/d s and didn’t explain anyone and she kept saying
Really!OMG :O
I was too embarrassed so left the room early.
WAS very depressed because of the examiners look of utter disgust !

FB: Passed with GS 4 so those who r going to appear dnt rely on examiners
look,attitude to judge how u did.
CZ the examiner on my 1st station looked very cheerful and I failed that.


S9.

Recent behavioural change in a old guy, admitted, CT done.Talk to his sister to
find out the reason and give d/d to examiner

asked all thq s regarding DEMENTIA mnemonic ,systemic review and Psych
qs,there was a positive h/O fall and family h/o behavioural problem,
FB:Passed

MVA q,I have seen someone posted it detail,so not repeating

I Messed it up ,didn’t even did the primary stuffs ABC checking,iv lines or fluids,

Just talked tp the pt that its secondary spleen rupture and will b needing surgery

FB:Passed (have no idea How :O but got 5 in approach to pt,and 2 in mx,so 1st
one saved me probably )

Guy with burning in pee.

on history all the findings for UTI was positive and features for BPH also for last
one year.
So explained with diagram what is going on,.


S12.Scrotal lump examination

I was very nervous when I saw the q bcz I didn’t have time to go through the day
before recall,so just read a geeky medics file some shared on a whats app group
for once.

Was having visible tremor when I was trying to wear the gloves but the exminer
was very helpful (I would give the credit to him that I passed ) he came forwardd
and told me,after all this years,it still happens to me, sp don’t u worry about that.
Then I laughed and it calmed me down

1st talked to the roleplayer,and then procced with inspection
Palpation-Temp
Tenderness
consistency of the lump could feel the testis or not
get above the swelling

Findings were very confusing and I rememberd in the hall that someone said its
varicocele on 9 feb recall,,but it didn’t seemed to me like that.and I couldn’t even
decide whether I can get above the swelling
I did trans illuminaion and it seemed positive
Checked hernial orifice

Then I said it could be hydrocele but I am not sure whether there is any lump on
the testis esp on the other side so I will do usg and see how things are actually

Had absolute no idea what to tell,did as a breaking bad news but failed, so better
to follow someone who passed

followed ASEPTIC with few organic qs and psych qs
Disoriented to time and place

Be prepared cause the roleplayer threw the tissue paper box at me and I was
about to fall down from the chair when he suddenly stood up

I prepared for a very short time like 3 and a half month to be total.
I wanted to take the 29 april but unfortunately bcz of system crush had to take
this date.So it was very stressful for me,had fallen very sick and even
hospitalized once,but Alhumdulillah,I made it.

I did selective cases from Karen and did 4 and a half month recall in total with
my two study partners,esp thanks to them.
Its best if u can do as much as like a year but I didn't have time.
I got a lot of help from this group and few of my seniors,I am really grateful to
them.So I tried to give the detail as much as I can .
