Working condition & injury narrative Statement –

My name is and I was employed for Fluor Inc. as a Waste Disposal

Labor with SAP number: 10439764. I worked in BAGRAM base for FLUOR from March 2017
until August 2018.

Working conditions

My regular working schedule is to work 12 hours a day, 7 days a week for a period
of approximately 120 days before taking R&R of approximately 15 days. I was assigned as a
labor to the Waste disposal department. My main duties were to execute the trash clean up and
disposal. I was mostly assigned to the Waste yard where I would assist in the waste sorting and
then disposal. I was doing my work on feet and in movement for the whole time. I had to pick up
and carry items very often. Most of the time it was waste that I needed to pick up, sort, or
dispose to waste point. The waste would weigh from 10-40 kg pending on the situation. In many
situations, I had to push the waste to the waste pit and use all my physical strength to execute
such tasks. In many situations when the security risk was higher, I had to work with the PPE
(helmet and vest) on me for the whole time. The PPE would weigh from 20-30 kg. I also had
many repetitive motions during my work, like bending and kneeling, picking up items, pushing
etc. The working environment was highly hectic and on the other hand we were constantly short
on man power. The demand from the management was constantly high and we had to deliver.

Injury narrative

Approximately in the beginning of June 2018, I started to experience a pain and

discomfort in my upper and lower back. I felt that I have pain, it was something that did not
really stopped me from working, but it really affected me. I wanted to give myself some time and
see how the whole situation will develop, but I had pain and had problems. I also did not find it
necessary to seek help from the medics, I was hoping that the pain would go away with time.

Approximately in July 2018, just before I was about to leave for my R&R, I also
started to experience pain and discomfort in my groin area, especially on the right side. My R&R
was close, so I did not want to raise concern or issue about this problem also.

 Approximately in the beginning of August 2018, while I was on R&R, the pain and
discomfort in my groin area was really affecting me and I needed to check that issue. I made a
visit to the doctor and I was suggested that I need to undergo a surgery on the inguinal hernia, on
the right side. I have undergone a surgery on the inguinal hernia on or around mid-August 2018.

Approximately since August 2018 along with the issue of inguinal hernia, I also
started to underwent examination for the problems that I am experiencing with my back. I have
undergone diagnostic screenings like MRI and EMNG on my extremities and the result showed
that I have problems with my upper and lower back. I am under therapy and treatment since then.
