1. I am a duly licensed driver in accordance with pertinent Land

Transportation laws, rules and regulations, and was issued a corresponding Non -
Professional Driver's License with number D16-05-301715 which is valid until January
13, 2008;

2. Sometime in October 2011, said driver's license was misplaced and got lost;

3. Efforts were exerted to locate said driver's license, but in spite of diligent search, it
could not be found and the same is now beyond recovery;

4. Said Driver's License has not been confiscated by the LTO, Police or other

Traffic Enforcers for any traffic violation;

5. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing

and to support my application for the issuance of a new Non - Professional Driver's
License, in lieu of the one that was lost;

discovered that it was missing;

5. The said passport was lost in the manner stated above and was not confiscated nor
cancelled by the issuing agency or other agencies for violation of any of its terms and
conditions, laws, rules and regulations;

6. I am now executing this affidavit to attest to the truth of the foregoing facts, to secure a
replacement of the said lost passport, and for all legal intents and purposes it may
serve.

sought the assistance of the Philippine Consulate in Kuwait, K.S.A. on how to return
to the Philippines without my Philippine passport;

4. With the help of the Philippine Consulate, I managed to travel from Kuwait, K.S.A. to
the Philippines using my travel documents in lieu of my passport;

5. The saidpassport was never used as a security to any obligation or

transaction;

6. I am executing this Affidavit to inform the Department of Foreign Affairs,

Philippines regarding the whereabouts of my passport and to request for a
new passport in lieu of the one that was withheld by my employer in Kuwait,
K.S.A.;

7. Finally, I am executing this affidavit to attest to the truth of the foregoing statements
and for all legal intents and purposes it may serve.
