ADDITIONAL REQUIREMENT FOR OB-HOSPITAL-VIRTUAL DUTY

SITUATION:

You were assigned at BGH-MC gyne ward and was assigned to a 65 year old, female,
married with 5 children, she belongs to the middle class family. Her admitting
diagnosis is "Ovarian New Growth Bilateral."

WHY DO YOU THINK THAT THE PATIENT WAS ADMITTED?

The 65-year old, female was admitted due to here crucial condition situation. Her Doctor
might be running tests and examination to confirm the diagnosis or there are other complications that can
be found. In order to know the cause and to treat patient with her condition, admitting her would be best
to closely monitor and evaluate any changes in her situation

QUESTIONS:

1. What symptoms are being manifested by the patient for her attending physician to admit her?

Many women have experience ovarian cyst. Most ovarian cysts present little or no discomfort and
are harmless. The majority disappears without treatment within a few months, but there are some
cases that will progress into ovary complication such as ovarian cancer. However, ovarian cysts
especially those that have ruptured can cause serious symptoms.

Symptoms are manifested by the patient causing her to be admitted in the facility. Sudden
and severe abdominal or pelvic pain (sharp pain is being felt if the cyst is large and about to
rupture.) Fever is also present with vomiting episode to the patient. Bloating and heaviness of the
abdomen is also one of the complaints of the patient (Fluid in an ovarian cyst can irritate the
lining of your abdomen, creating inflammation and inflammation can cause bloating.)

2. What possible diagnostic test does she need to undergo.

There are 2 commonly use diagnostic test for suspected ovarian cyst showing probable signs in
addition to complete pelvic exam. Transvaginal ultrasound (TVUS). A transvaginal ultrasound is
an internal examination, the procedure involves the insertion of the transducer into the vagina to
produce incredibly detailed images of the organs in the pelvic region. CA- 125 blood testing is
also to be done to patients with high risk to measure the amount of the protein CA 125 (cancer
antigen 125) in your blood. A CA 125 test may be used to monitor certain cancers during and
after treatment. CA 125 test may be used to look for early signs of ovarian cancer .
3. Give 2 nursing diagnosis for the patient.

 Anxiety r/t surgical procedure as manifested by fear, restlessness, and BP of 140/90: presence of
flushed skin and dilated pupils
 Fear related to hospital procedures and treatment as evidenced by the client‘s verbal report that
she might die during the procedure.

4. Make a sample charting for this patient who is admitted to the ward.

patient to express fear and concern. Educated patient about her current
condition. Explained the procedure that will be done. Educated the
patient for possible outcomes of the surgical procedures to be done.
R: “Ngayon mas naintindihan ko na yung sakit ko, nabawasan kaba ko
kasi buti na lang pumunta ako agad sa Ospital para maagapan.” Patient
reported understanding about her condition and procedures that can
possibly be executed to her.
