Please check the services desired:

Standard DJ Services Include: Mixer; Amplifier; Two 150 Watt Speakers; One Microphone on
ten foot Cable; Basic Lighting System.

$200.00 for the first two hours and $100.00 per hour thereafter with a three hour minimum.
A $150.00 overtime will be charged for additional hours not specified in this contract.

ADDITIONAL SERVICE RATES:

A 10% deposit is due at the signing of this contract, payable to Julian Hayda.

Terms of Agreement

1. Minimum deposit of 10% required to bind terms. Check should be payable to Julian
Hayda.
2. 50% of the deposit will be refunded if client cancels more than 30 days prior to the event.
3. Parties entering into this contract are permanently bound to fulfill the obligations by this
agreement unless one or more of the following conditions apply:
a. Both parties mutually agree to leave the agreement and its responsibilities.
b. If the event described is cancelled or rescheduled a minimum of 30 days notice is
required or there will be no refund of the deposit.
c. Full balance will be due within 24 hours of the event.
4. In the event that DJ Julian becomes unable to provide his services due to complications
beyond his control, a reputable replacement shall be provided at no more than the
 original price agreed on. If this is the case before a replacement is made available, a full
refund will be provided.
5. Any and all information or details must be communicated before the 5 days preceding the
event.
6. Julian Hayda shall not be held liable for any action arising from or in connection with any
individuals not directly affiliated with Julian Hayda. Furthermore, the party(s) contacting
DJ Julian agrees to assume full responsibility for any and all damages caused by
themselves or their guests involving any of the DJ Julian property.
7. The parties purchasing the services of DJ Julian agree to allow DJ Julian to use pictures
and video images taken at this event for promotional purposes

Client shall ensure that: (A) DJ is provided with a sturdy, covered 6' or 8' DJ table within 25' of a 3
prong grounded electrical outlet; (B) the place of engagement is open and available to the DJ at
least one (1) hour before the scheduled starting time; (C) the place of engagement meets all state
and federal safety regulations; (D) for all outdoor performances, unless waived by the DJ, a tent
or similar overhead protective covering must be provided to prevent damage to the DJ's
equipment in the event of adverse weather conditions; (E) reasonable steps will be taken to
protect the DJ and his equipment & music collection from any type of abuse, theft or damage
resulting from this booking. In the unlikely event of circumstances deemed by the DJ to present a
real or implied threat of harm to the DJ's equipment, music or person, the DJ reserves the right to
cease performance until the Client has resolved the threatening situation. (F) The DJ reserves the
right to deny any guest access to the DJ's music and/or equipment.
