PART II SECTION 1: INTRODUCTION AND GENERAL REQUIREMENTS


1.1 GENERAL
The policy of PT Paiton Energy (“PE” or “Company”) is to "buy in" the technical
capability necessary to undertake the required works.
This specification will not seek to identify the method that the successful Contractor
should adopt, but identify the areas of work and the scope of the requirement.
It is the Contractor's responsibility to identify how the work should be carried out to
meet all requirements of the Specification. To this end the Contractor will be required to
submit at the proposal stage, a method statement that explains fully how it will
undertake the specified work. Failure to submit a method statement may exclude an
otherwise acceptable proposal from consideration.
Acceptance of a Contractor's proposal does not mean that Company assumes any
responsibility for the success of the Contractors intended method(s). No additional costs
will be in the event that an intended method requires amendment or modification to
meet the requirements of the Specification.
Should Company request changes in any proposed method prior to acceptance of the
proposal, and the Contractor agrees to such a revised method, then it will assume
responsibility for the success of that method.
As the Contractor is detailing how he will meet the Company’s objectives there shall be
no undisclosed "extras", or additional costs as the original quoted sum will be the only
amount paid.
The above statement is not meant to circumvent the "Change Order" which will still be
used where additional work or a change in the scope of the work is expressly agreed to
by the Company.

1.2. METHOD STATEMENTS


The Contractor must complete Schedule 4.4 supply as part of their proposal with the
following information.

Method Statements detailing how the specified Works will be carried out and including
samples of the reporting format(s) to be used. The Method Statement must provide
sufficient detail of the Contractor’s intended designs, material selections, implementation
processes or procedures, testing procedures etc. to the extent necessary to explain to
Company how the scope of work requirements will be met.

Risk Assessments for all activities associated with carrying out the specified works. In
particular, the Contractor shall detail (i) how they intend to carry out the Work in full
compliance with the specified Health and Safety requirements (compliance with which is
mandatory), and (ii) how they intent to mitigate any reasonably foreseeable risks that
might adversely impact timely completion of the Work.

Environmental Risks any environmental risks caused by carrying out the specified works
must be identified and addressed with particular emphasis on unauthorised discharges
into the atmosphere, the surface water drainage system, or other watercourses.

1.3. PROGRAM / SCHEDULE


Contractors must complete Schedule 4.5 and submit with their proposal a program
giving timescales for each element of the work specified, and either confirm or
demonstrate that they can meet the schedule requirements as detailed in section 2.3.

1.4. CONTROL OF HAZARDOUS MATERIALS


Contractors must complete Schedule 4.6 and provide Material Safety Data Sheets for all
hazardous materials and substances they propose for use in the execution of the
specified work.

1.5. QUALITY ASSURANCE / QUALITY CONTROL PLANS


Contractors must operate to a Quality system that complies with one of the following:
(a) ISO 9001 - where the scope of the Contract includes design/development,
production, installation and servicing.
(b) ISO 9002 - where the scope of the Contract includes production, installation
or site refurbishment work.
If a Contractor believes their system complies with an equivalent standard,
agreement by Company will be required.
Company only recognises third party certification schemes accredited in compliance
with the standard by recognized national or international Accreditation Bodies.
The Contractor shall submit with the Proposal evidence of ability to comply with the
required quality system standard and/or shall supply details of contracts where the
quality system has been used successfully relevant to the intended works to be
performed under the Contract.
Contractor will be required to submit a final Quality Control Plan (“QCP”) prior to
commencement of the Work.
During the Contract, Company may monitor the quality system by surveillance of
work and by formal audits of the Contractor's quality assurance arrangements. Any
corrective action identified shall be agreed and implemented.
For in-works requirements, the Contractor shall provide access and facilities required
by Company for monitoring activities.
Company reserves the right to participate in the Contractor's monitoring of sub-
contractor's quality systems.

1.6. OTHER WORK/ DEPARTURES FROM THE SPECIFICATION


Contractors may at their option complete Schedule 4.7 and submit details of "Other
Work" or alternative procedures that they may wish Paiton Energy to consider. It is

essential that any "Other Work" offered is in addition to the specified work for which
prices must be quoted in Schedule 4.2.

In the event that the Contractors proposal departs in any non-material way from the
specified Work, then full details must be included here. Such departures might include
reference to alternative, but internationally recognised, equivalent standards. Failure to
document such departures in this Schedule will mean that the Contractor is fully
compliant with all Company’s stated requirements.

1.7 CONTRACTORS STAFF ORGANISATION


Contractors must complete Schedule 4.8 submit a Staff Tree for their organisation
relevant to the operation of this contract. This should show all levels of management
and supervision and identify nominated staff for day to day contact for the execution
and progressing of work. Out of Hours contact names/telephone numbers should also
be included.

1.8. SUB CONTRACTORS


Contractors must complete Schedule 4.9 and provide details of any sub-contractors that
they propose to use in the execution of the contract. Paiton Energy reserves the right to
approve sub-contractors.

1.9. EQUIPMENT, MATERIALS, TOOLS AND LABOUR


The Contractor shall supply all Equipment, Materials, Consumables, Labour, Supervision
and all other items or services necessary to complete the Scope of Work within the
agreed Schedule with the exception of those items referred to in section 2.4.

1.10 RECOMMENDED SPARE PARTS AND CONSUMABLES


The Contractor will complete Schedule 4.10 and as applicable provide a priced list of
spare and consumable items as follows:
(i) Consumables. Those items that will be required for operation and planned
maintenance of the Plant supplied for a period of one year from Taking over by
Company. (NB: The Contractor is responsible for the provision and cost of all
items necessary for commissioning of the Plant).
(ii) Spare Parts. Those items that may be required during the first two years of
operation and replaced at planned maintenance intervals.
(iii) Breakdown Spares. Those items (other than those already included in (ii)) that
the Contractor advises holding to meet breakdown situations. These will usually
be long lead-time items.

The cost of the above items will be taken into account during the assessment of the
Contractors proposal.

1.11 TECHNICAL DATA SHEET(S)


The Contractor shall complete Schedule 4.11 and supply information relating to the
performance of Plant or Equipment he proposes to supply. This shall typically include:
Dimensions
Materials
Operating Conditions (Temperature, pressure etc).
Operating principles
Input requirements (electrical, fuel type etc)
Consumption (normal and maximum)
Output conditions (power rating, voltages, emissions etc. Normal and
maximum)
Terminal point data (measurements, connection type and performance criteria)
Control systems (type and operating principle. Software standards etc)

The Contractor shall complete Schedule 4.12 providing at least three similar examples
of similar projects completed with the last three years.

1.13 PRICES
Contractors must complete Schedule 4.2 "Prices" by quoting fixed lump sum prices for
each element of the work specified.

1.14. SCHEDULES
The Schedules in Section 4 must be completed. Assessment of proposals cannot be
undertaken without this information. The Contractor may extend any Schedule by
adding additional pages or including (and clearly referencing) additional documents.

1.15. SITE VISIT


All Contractors are required to make a site visit to examine the installation and view the
drawings and documents quoted in this specification prior to submitting a proposal. The
Contractor shall supply all necessary Personal Protective Equipment for this site visit.

PART II, SECTION 2: THE WORK

2.1 PLANT DESCRIPTION AND LOCATION OF WORK

Some locations in Paiton Unit 3 is identified need some additional lamps. Those locations are
west side of chlorination building, trash rack system toward intake canal, right side of main
gate, water treatment plant SWRO module area and battery room. West side chlorination
building is required as a back-up when there is a problem on the lamp located at 500kV
transformer yard gantry. On the trash rack system is required for illuminate their intake
canals. Right side main gate is required for traffic warning lamp. Water treatment plant
SWRO module is required to re-arrange the power source to prevent very low illumination
when there is a trip or breaker any breaker isolation for maintenance works. In battery room
since there is possibility H2 accumulated on the room due to battery reaction so the explosive
proof type of fixture lamp shall be used as a replacement of the standard type of existing
fixture. There is also some switch and socket on the room need to be replaced with the
explosive proof type with hazardous area classification Class 1 Div.1 group B.

2.2 SCOPE OF WORK DESCRIPTION, OBJECTIVES AND DELIVERABLES


2.2.1 Scope of Work
The scope of work is to provide additional lamps and all the required materials to
complete the scope of works. The lamp shall be from saving energy (LED) type or LED
lamp. The cables requirement will be provided by POMI.

2.2.2 Objective
The objective of the project is to provide enough illumination level and use of the suitable
lamps fixture to the locations as mentioned on the scope of work.

2.2.3 Deliverables
The contractor shall provide saving energy (LED) lamp type with all the required
accessories (fixtures, post or fixture holder, conduits) and perform site installation
following the existing facilities.

2.3 SCHEDULE OF WORK (PROGRAM)

The overall works shall be completed on July 2020.

PAITON ENERGY (IPMOMI) SCOPE OF SUPPLY

Paiton Energy (IPMOMI) will supply only the following as part of meeting the overall
requirements for completion of the Work:
2.4.1 Permit for Work.
2.4.2 Provide safety induction.
2.4.3 Power cable for the lamps.

2.5 CONTRACTOR SCOPE OF WORK

In order to meet the deliverable of SOW briefly stated on section 2.2 the following are contractor
scope detail as minimum but not limited to:
2.5.1 Perform detailed study of existing situation and conditions, in order to determine the
most suitable scope of supply.
2.5.2 Provide all material requirements to complete the scope of work but not limited to
the followings: lamp, fixtures, post or fixture holder, conduit and its required
accessories see the related Bill of Material (BOQ) for reference.
2.5.3 The lamp shall be Philips saving energy (LED) lamp type. Bidder shall provide the exact
LED lamp type including its catalogue consist of illumination diagram of the proposed
lamps on their proposal for owner review and evaluation.
2.5.4 All the cable shall be run inside the suitable size of galvanize conduit type as the main
and some flexible conduit at the bonding point or whenever will inside to the equipment
except for underground cable or if along the route there is a cable tray available.
2.5.5 Provide all the required tools to perform and complete the scope of works.
2.5.6 The new lamp source shall be fed from the adjacent availability of similar source
lamp type.
2.5.7 Provide temporary access platform (scaffolding) suitable for installation works.
Contractor is responsible to determine the total number of scaffolding quantity
requirement.
2.5.8 Performed site jobs installation and commissioning to complete the scope of works.

2.6 RISKS IDENTIFIED AND HIGHLIGHTED

The work will consist of the following:


 Work on height during installation and commissioning so suitable work method shall be
applied.
 Work near the electrically energized equipment’s so suitable work method shall be
applied.
 Work near the noisy and high temperature environment so suitable work method and
related PPE shall be applied.

2.7 DESIGN LIFE AND ENGINEERING STANDARD

The design life of new system shall comply with common Industrial Engineering
Standard of the similar industry and application refer to applicable recognized
Engineering standard and code.

2.8 NOTIFICATIONS AND SPECIFIC REQUIREMENTS


The following are specific considerations and requirements relating to this Work. As
applicable the Contractor shall demonstrate that it can comply with these standard
and requirement:
2.7.1 Contractor shall notify PT Paiton Energy in writing as soon as it becomes aware of
any change, or potential change that may affect schedule (program), material

specification, installation techniques or standards, commissioning activities,
witnesses, testing etc.
2.7.2 Company shall inform contractor in regard to any change in the Scope of Work, any
Plant and environment condition, or other event that may adversely affect the
Contractors work progress or provide additional safety requirements.
2.7.3 Any events occurring under either 2.7.1 or 2.7.2 above shall be dealt with in
accordance with the Change Order provisions of the Terms and Conditions of
Contract.
2.7.4 The equipment that will be used to deliver the scope shall have valid certificate.

2.9 STANDARDS

In addition to the standards and specifications specified in the Contract, the


Contractor shall also comply with the following:

2.9 COMMISSIONING AND TESTING

2.9.1 The following items are subject and mandatory for inspection: The functionality of
the complete system after modification completed.
2.10 FINAL COMPLETION AND ACCEPTANCE CRITERIA

Completion will be defined as the date on which the Contractor has completed all
requirements of the Work (including satisfactory completion of all tests) and
demonstrated to PT Paiton Energy’s satisfaction that the Works perform as intended and
meet all required standards and acceptance criteria as stated on section I and II by
IPMOMI and applicable Quality Control documents. Supply of all manuals, material or
test certificates, documentation and drawings is a condition precedent to declaration of
Completion.

2.11 TAKING OVER PRIOR TO FINAL COMPLETION

2.12 DOCUMENTATION AND REPORTS ON FINAL COMPLETION

2.12 As applicable, the Contractor shall provide 3 copies of any specified reports. The
report(s) submitted shall take the form of a written report, with a copy on standard
CD/DVD in Word and/or Excel as appropriate.

2.13 Digital photographs shall be provided in CD-ROM format, in addition to hard copy
versions. Resolution shall be suitable for the intended purposes.

2.14 Contractor agrees that Paiton Energy may use all drawings and documentation supplied
to maintain, or have maintained, the completed Scope of Work.

2.13 CERTIFICATE OF COMPLETION

When contractor has demonstrated to the company that the works are complete in
accordance with the contract the Company shall issue the Certificate of Completion
(CoC) documenting the works are taken over by the Company. The Certificate of
Completion shall accompany any application for final payment.

The following documents and drawings are attached to this Request for Quotation:
The contractor is deemed to examination and verifies other information supplied for the
validity and accuracy where applicable. Company does not warrant that the drawings are
accurate or can be rely upon.
