Diagnostic report and recommendation
As you (my patient) are 58 years old and you are a
continuous smoker for past 30 years due to which you are the patient of hypertension. The second point which is to be noted is that you work a lot due to which you are suffering from low back pain.
My Recommendations Being a Doctor:
About your poor sleep:
Firstly the point to discuss is your poor sleep, due
to which your body’s whole physiology has been disturbed. My first recommendation to you is to fix a proper time for your sleep which should be maximum of 7-8 hours Due to not getting enough sleep your body is unable to work properly About Weight loss and Healthy Eating:
Second point to discuss is your body weight
As you are overweight due to which you are the patient of hypertension (increased blood pressure) Now here my aim is to address you about the benefits of exercise and healthy eating which will help you in a long run : Now see yourself and observe, being overweight has caused so much trouble for you, you are not able to sleep properly, you are suffering from high blood pressure, you are suffering from back pain, and due to increased level of cholesterol in your body you may develop cardiovascular diseases. As cardiovascular disease is a disease which affect your heart and arteries due to fat deposit in them. So now if you want to reduce the risk factors of CVD you have to exercise regurarly along with healthy eating If you will exercise daily it will help you get rid of your stored fatty cells which will help you maintain good blood pressure So daily you have to exercise for 30 mins, you can walk , you can do brick walking , you can do yoga or meditation Try to avoid fatty foods like burger, pizza, pasta, noodles and food rich in carbohydrates. Here is the diet plan which you can follow to start the habit of healthy eating And try to quite the habit smoking it will cause worse effect on your lungs. I know you cannot quit it gradually. If you are smoking 3-4 cigarettes in a day try to smoke less like 1 or 2 and then try to reduce it to 0 if you will not quit it your lungs will shrink and will stop working so try to quit this habit as soon as possible. About Back pain: As you have to work for longer period of time and you have to sit in a particular place for many hours. Your low back pain is the symptom that you are obese and you are not following a proper posture. Here are some recommendations which will help you reduce your back pain: Maintain correct posture DO stretching Maintain a healthy weight Use Lumbar Sport
Here is the chart of some simple exercise which you can
do to reduce lower back pain you can perform it with trainer or once you are able to do it alone you will I assure you if you will follow all the recommendations your body’s physiology will come to normal and you will be able to spend your life in a healthy way reducing the risks factors of diseases.
Will he adhere to my Recommendation:
Yes, I am sure that he will adhere to my
recommendations because firstly the way, in which I have addressed him about the good and bad of all things, the way I have talked to him is very much important for a doctor to help his patient adhere with your recommendation. Secondly he was so serious or conscious about his health that he thought of consulting a doctor.
What are the applicable incentives and penalties
complying not with the guidelines
Incentives are really important in health care
profession. They help to improve the modes and methods to get the maximum result/outcome from the patient. Incentives encourages patient to perform well. Incentives may be monetary and non-monetary If you will tell your patient that he has to see you once in a week and you will take look of his health condition or you will check how well he is doing so by doing this there are chances that your patient will improve well. Penalty for the patient for not performing well is that you will not be able to improve, the more the situation get worse it’s difficult to handle it, maybe you (patient) have to be admitted in the hospital for not following the recommendations. NOTHING LOOKS AS GOOD AS BEING HEALTHY FEELS
