Talisay City, Philippines, do hereby CONSTITUTE, NAME AND APPOINT, ERLINDA
D. ALAYON, of legal age, Filipino, and resident of Purok 7, Poblacion, Compostela,
Davao de Norte to be my true and lawful attorney, place, and stead to do and perform
the following acts, and things, to wit:

“To claim my BDO Network Bank ATM Card in my behalf.”

“To sign and execute my document and papers on my behalf with regards to
my ATM Card.”

HEREBY GIVING AND GRANTING, unto my said Attorney-in-Fact, full power

and authority whatever requisites or necessary or proper to be done in and about the
premises as fully to all intents and purposes as I might or could lawfully do if personally
present with power of substitution and revocation and hereby RATIFYING AND
CONFORMING all my said attorney of their substitute shall lawfully do or cause to be
done under and by virtue of these presents.
