A.p Xnowledge of the Disease

CVD bleed is a cardiovascular disorder where occurs when a blood vessel


ruptures and bleeds into surrounding tissue. Infarction is a blockage of a vessel in
the part of a brain which causes decrease supply of oxygen to the brain. The factors
which trigger this condition are smoking, high cholesterol intake, alcoholism and
aging.
Pulmonary tuberculosis (TB) is a contagious bacterial infection that mainly
involves the lungs, but may spread to other organs. You can get TB by breathing in
air droplets from a cough or sneeze of an infected person. Risk factors include Are in
frequent contact with people who have the disease, have poor nutrition, and living
in crowded or unsanitary living conditions.

B.p Proper Hygiene / Sanitation

Wash and dispose properly all the articles came in contact with the client.
Sterilize it if possible. Wash hands properly especially before and after eating. Take
a bath daily. Wear clean and gentle clothing. Observe proper cares such as of
mouth, eyes and ears. Xeep environment clean. Provide adequate ventilation. All of
these practices could help prevent spread of further infection.

C.p Nutrition

Acquire adequate nutrition, through observing proper diet, eat complete 3 meals
a day. Avoid high salt, high sodium intake. Have at least 8-10 glasses of fluids daily.
Have a vitamin and mineral supplementation.

D.p Medication

Take Losartan 100mg 1tablet OD, Ranitidine 150g 1tablet twice daily, Lactulose
30mL OD @ hours of sleep, Isosorbide mononitrate 60mg ½ tablet once daily,
Simvastatin 40mg 1tablet once daily, Acetylcysteine 600mg 1tablet once daily
dissolve in half glass of water, Myrin P Forte 2tablets once daily, Clonidine 75mg
1tablet every 6 hours. Don͛t skip doses, and continue taking as indicated by the
physician.

E.p Follow-up Check-up

Establish regular check-up, and have a consultation 1 week after discharge, to


assess for further progress or complication.
