AFFIDAVIT OF CONSENT FOR

EXHUMATION, CREMATION AND RE-
INTERMENT

We, NEIL CLARK M. ARAGON, LLOYD JEFFREY M. ARAGON

and JAN CARLO R. MONTEALTO all of legal age, Filipino citizens and
residents of 11 Laurel Street, T.S. Cruz Subdivision, Novaliches,
Quezon City after having been duly sworn to in accordance with law
and state that:

1) We are the children/heirs of CARIDAD REBECCA M.

ARAGON (Deceased) who is separated-in-fact from
VICTOR H. ARAGON (Spouse);

2) Our mother died on July 8, 2020;

3) We agree that the remains of our mother CARIDAD

REBECCA M. ARAGON will be interred at Lot H – Upper
Tier, Manila Memorial Park - Holycross;

4) We hereby give our full consent for the exhumation,

cremation and burial of our mother CARIDAD REBECCA
M. ARAGON to Lot H – Upper Tier, Manila Memorial Park -
Holycross;

5) We hereby undertake and declare to agree that in the

event somebody will question or claim the bone remains of
our parents who were buried in the aforesaid Memorial
Park, we will be answerable and liable and further declare
that the QUEZON CITY HEALTH DEPARTMENT will be free
from any liability;

6) We hereby dispersed that Manila Memorial Park -

Holycross shall not be liable to any person who may
present a lawful claims on the above transaction we have
undertaken and to indemnify any such person who shall
make a lawful claim on the same;
 7) We are executing this affidavit to attest to the truth of the
foregoing facts and to support our request with Manila
Memorial Park, Quezon City to allow the exhumation and
re-burial of our mother for whatever legal purpose this
affidavit may served;

8) We will process and attend to the necessary

documentation of acquiring a permit to your office.
