Efficient and architecturally designed for specific needs of school nurses and community members, this building type is urgently needed in the time of coronavirus as Alabama's school children return from quarantine. It will assist as so continues under new social distancing guidelines. The small satellite building keeps children safer than if infected student patients are in a standard nurse’s office in the main school building which is connected to the main air handling system. Auburn School of Architecture has a long history of successful and high impact public interest design, research and outreach. This expertise can be applied to the repeatable design of the medical clinic. Sets of plan options for differing site types and construction specifications will be developed and designed by the team. Each public school will be provided with plans that can be used by local contractors for quickly building the units statewide. Our researchers will conduct a sampling of accountability audits at construction sites in each county to observe quality control of construction and building performance. ‘Auburn School of Architecture’s Front Porch Initiative has developed replicable small house designs that are durable, resilient, energy efficient, and built locally to last 100 years. This effort has focused on small rural housing units in underserved communities. Extensive research into construction details, materials and energy efficiency have been tested in the process of designing and building over 20 house/prototype versions in West Alabama through the Rural studio These already developed details (such as foundation and wall section options) may be adapted to this clinic type and condense the time frame from construction to occupancy. The Front Porch initiative has designed a series of detailed specifications that local contractors can build from using readily available commercial building materials. Design: While outfitted with industry standards in healthcare materials and equipment, the comforting aesthetics of the small design are welcoming to young children. The space provides parents with a specific place to go to pick-up their children without the children having to go back through the main school building potentially spreading the illness. Each efficiently designed facility includes a nurse’s office, isolation units for student patients who may have COVID-19 or influenza, and treatment rooms for other ailments and dispensing, medication. It is essential that the building has a separate HVAC system from the main school building in order to control transmission of illnesses through the systems. Students will go to the clinic until a parent or guardian is able to come get them and transport them to a doctor. Architecture: This Medical Clinic design has a long life span and higher quality of construction and healthy materials for medical application than a portable building or FEMA trailer which both have a shorter life span. Alabama's investment in these structures will assist the health of school children over several generations. Additional Uses & Impact: The facilities built all over the state will also be available at times for community medical needs including testing. Local contractors will benefit from the opportunity to build these small units given the state of the construction economy during the pandemic. Schools have an important role in slowing the spread of diseases to help ensure students have safe and healthy learning environments. Schools should be prepared for COVID-19 outbreaks in their local communities and for individual exposure events to occur on their campuses. ‘The Centers for Disease Control (CDC) has developed multiple guidelines for the safe return of students to the school setting. The guidelines include promoting behaviors that reduce the spread of COVID-19 such as to: ‘a. “Teach and reinforce handwashing with soap and water for at least 20 seconds and increase monitoring to ensure adherence among students and staff.” b. “Support healthy hygiene behaviors by providing adequate supplies.” “Ensure ventilation systems operate properly and increase circulation of outdoor air as much as possible, for example by opening windows and doors. Do not open windows and doors if doing so poses a safety or health risk (e.g., risk of falling, triggering asthma symptoms) to children using the facility.” d. “If feasible, conduct daily health checks (e.g., temperature screening and/or symptom checking) of staff and students.” e. “Immediately separate staff and children with COVID-19 symptoms (such as fever, cough, or shortness of breath) at schoo!” and “identify an isolation room or area to separate anyone who has COVID-19 symptoms or tests positive but does not have symptoms.” “School nurses and other healthcare providers should use Standard and Transmission- Based Precautions when caring for sick people.” ‘The National Association of School Nurses (NASN) advocates a space for isolating any student with a suspected infection of COVID-19. NASN notes schools should “keep sick students and staff, particularly those with symptoms of respiratory illness, separate from well students and staff until they can leave.....(and should) plan to have areas where these individuals can be isolated from well students and staff until they can leave the school.” Furthermore, schools should “evaluate the current school nurse designated space and determine if there isan adjacent space for isolation.” The organization stresses, “If an adjacent space is not available, consider moving the school nurse work area to another larger location with a separate adjacent space. Consideration of ventilation such as windows and an outside door is preferable to reduce the spread of disease for isolated individuals exiting the building. Computer, phone, internet, and restrooms with handwashing facilities are required in the school nurse designated space.” ‘The ASN surveyed all 138 school district Nurse Administrators/Lead Nurses in Alabama with an 83% response rate. Responses reveal the following abysmal data: * 17.63% of schools DO NOT have a nurse on campus throughout the day © 67.83% of Nurse Administrators/Lead Nurses are assigned to care for students in a school in addition to their district wide administrative duties 40% of districts report they DO NOT have enough face masks for their nurses 88.7% of districts report they DO NOT have enough pediatric face masks for students 47.83% of districts DO NOT have enough gloves for their nurses 97.39% of districts DO NOT have enough gowns for their nurses 88.7% of districts DO NOT have an isolation area 75.65% of districts DO NOT have sink in every nurse office 91.3% of districts DO NOT have a toilet in every nurse office ‘80% of districts DO NOT have thermal (no contact) thermometers ‘The Coronavirus Aid, Relief, and Economic Security (CARES) Act funding for Alabama schoo! nurses will be used to provide appropriate and safe isolation areas for students and staff who exhibit COVID-19 symptoms as outlined in CDC guidelines and NASN recommendations. It will provide sinks and toilets in the nurse offices, which are currently lacking but vital, for the most basic hygiene practices. The funding will provide minimally adequate levels of Personal Protection Equipment (PPEs) necessary for the protection of nurses and students alike. The funding will provide thermal scanning capabilities which will increase screening and nursing assessments with reduced potential for tactile transmission of the virus. School nurses should be considered leaders of health promotion and disease prevention in schools. School nurses provide assessments, culturally sensitive care and interventions, and health education with the goal of decreasing fear and promoting the prevention of COVID-19, the flu, and other illnesses. Without the CARES Act funding, Alabama school nurses will be tunable to fulfill their duties. My district is very rural and underserved because of our limited resources. 1 believe that this plan will help us address, to some degree, that inequity and allow my constituents to feel comfortable that they are receiving the same support to reopen their schools as the more affluent districts of our state. This is extremely important to my communities as they lack school nurses and other critical health access. I believe we have seen the best possible plan for providing a degree of confidence in parents, students, and faculty and support personnel that our schools will be as safe as possible whey they reopen this fall The isolation and quarantine room is certainly an integral part of that plan. The questions are of course: Can we have them in place in time? Here are a few options 1. One standard plan that is approved by the Building Commission saving thousands of dollars in architectural fees, Establish a penalty if the contract does not have the IQ Units ready by the start date, 3, Ask Dr. Harris to strongly recommend that no school begins until they have an 1Q room Have schools designate a temporary isolation room until the IQ Unit is completed. Please sce the current Alabama Competitive Bid Law that exempts construction projects that do not exceed $50,000.00, There are two options for construction that can be finalized to ensure that the IQ Units do not exceed that threshold. I believe that we can bid them individually by school as multiple contractors will likely be working simultaneously. With this proje: ime is of the essence in order for construction and acquisition to be complete by August. Stand-alone Medical Clinic Provide every school architectural plans with a stand-alone medical clinic built outside the school structure (designed by Auburn School of Architecture) with a trained and licensed nurse and test every student for COVID-19- Influenza A & B. Cost estimates include all necessary medical equipment and supp! Structure will have self-contained ventilation system, Children identified with an illness can be dropped off and picked up by parent without centering the main school facility Each structure will contain an isolation unit. Local contractors and suppliers will be used to promote economic recovery in that area under strict quality control. Smart Thermography Entrance Active monitoring and isolation of students/staff suspected of having COVID-19, and it will provide each school with a testing machine for quick results. Expected scan time is less that 3 seconds per student as they enter the building. ‘Thermal cameras can detect the small temperature rises using a thermal reading of the tear duct (cartiod artery) providing notification to a secondary screen. can also detect other COVID-19 symptoms including anomalous treating and heart rate, hypoxia, coughing and conjunctivitis, * Provides social distancing measurement, awareness and statistics. Safely Opening Schools: SOS Program Facts All Staff including teachers, coaches, lunchroom workers, etc. * Software will be provided for all staff to spot check students throughout the day. This program provides the software and they can use their annual stipend for 2021 school year to purchase hardware to match their mobile device. Bus drivers with an older student helper—small wifi tablet-— * Acblue tooth wireless thermal device that would link up to the software. This device would use a forehead scan with more accuracy than the non-medical infrared devices ‘most are using today. + Tablet or Ipad would be preloaded with the students names and the driver or his assigned “helper” who can check for temps, identify students who exceed 100.4 and place them in a designated seat ~ modified by removing one row of seats and using a poly vinyl curtain/barrier. * The software would alert via email the nurse that that student will be coming once the bus is back at the school or within range for data notifications. * That student would go directly to the school nurse [QUnit to be reviewed. For roughly the same amount of money ($150 million) ADPH has to spend to test about 2 percent of the population monthly over the next 30 months, the SOS program has the ability to test and report results statewide on a single platform. Over 15 percent of the entire population can be found within a public school on a Monday- Friday during the school year. We are not asking for the full $150M that ADPH has to do this task- just $26M and we could provide 500,000 results in 9 months helping ADPH to meet their objective. ‘The cost of providing a school nurse for each school is funded through CARES ACT funds for the first year. However, federal monies could be available to fund those costs - or a portion of those costs -- for many more years. Regardless, a full school year of interaction could literally change the lives of thousands of Alabama school children. Safely Opening Schools: SOS Program Longterm Benefits * Provide each school (1,400) with medical units for student/staff to identify and isolate students with contagious diseases and injuries both now and in the future. + HIPPA Blind results from students’ tests will be sent to ADPH and CDC for data collection. * Provide 500,000 test results in 9 months helping ADPH meet their objective. * Some 87 percent of costs for the first two years would qualify for CARES funds, * Higher quality used in the construction will have a longer life than a portable building or FEMA trailer allowing them to be used for several generations. * The testing equipment can also be modified to test for other ailments for future outbreaks including mumps, measles, chicken pox, ete. * Provide all schools with an operable first-aid structure (including an isolation room) to treat sick and injured students both currently and in the future, Many schools lack an area for nurses to treat and care for students. A large number of areas designated for nurses lack even running water. * Puts nurses in schools where impoverished children rarely interact on a regular basis with a healthcare provider of any kind. * Nurses are trained to spot signs and symptoms of more serious issues involving mental health, abuse, drug use, and can catch issues early. * The economic development benefits from this program could generate more than $100 million, most of which will go into local communities — including some that are in desperate need of new projects. Jobs in local small owned business, utilizing skilled, semi-skilled and unskilled labor throughout our state. * The ability for school districts, local leadership and others to have open house/ ribbon cuttings throughout the state prior to school starting will also serve to reassure parents that everything possible to keep their children safe and healthy is in place. Getting, students in schools will help parents return to work, Schools participating in the SOS program- at no cost to the system- would get: > $50,000 for the construction of a 500 sq ft IQ Unit at each school > $7,500 per IQ Unit for startup PPE and supplies. > 1nurse per school if that school did not have a full time nurse in 2019-2020 > 1 Thermal screening solution (either fixed mount at entrance or on a mobile kiosk) includes installation and software license/ support for 2 years. ® 1 Sofia 2 SARS Antigen Fluorescent Immunoassay system- with wifi connectivity to report results to ADPH or CDC. © 10% of last years student enrollment per month in Influenza A+ Band Covid tests.
