IMPORTANT NOTE: This report has been designed to enable you to objectively appraise the work
performance and potential of managers and associates in your section or department. Please keep in
mind that the appraisal will become an important part of the Employee’s record and will influence
his/her future development and career. It is therefore important that you consider all of the following

Meets job requirements : 3–4
Meet reasonable and realistic job requirements and often exceeds them. Is effective with only
occasional Supervision.
Better than required standards :5–6
Consistently exceeds requirements and standards. Highly effective with only general guidance
required substantially above expected standards.
