Definition
This refers to the cumulative number of organized contact tracing teams.
This refers to the cumulative number of individuals in the CTT.
This refers to the cumulative number of individual members in the CTT
trained in contract tracing.
This refers to the cumulative total number of laboratory-confirmed COVID-
19 cases
This refers to the cumulative number of confirmed cases which have been
successfully traced.
This refers to the percentage of the cumulative number of confirmed cases
which have been successfully traced.
This is the cumulative number of close contacts of a confirmed case who
have been listed
○ b) face-to-face contact with a confirmed case for at least 15 minutes, OR

Assessed would mean that the Close Contact was interviewed either via phone or through other messaging
applications, or by face-to-face interview, and the health status and disposition were determined.

listed which have been successfully traced and assessed.
This ratio refers to the number of close contacts traced and assessed
against the total number of confirmed cases traced.
This refers to the number of Confirmed Cases in non-health isolation

The source of the data is from the DOH-Centers for Health Development. Data sources would not automatically be
generated from the contact tracing efforts, as there are other sources of infection. The sum of Columns S and V
should be added on column E. But the sum of columns S and V should not be equal or greater than column E.
Confirmed case - any individual, irrespective of presence or absence of clinical signs and symptoms, who
was laboratory-confirmed for COVID-19 in a test conducted at the national reference laboratory, a sub-
national reference laboratory, and/or DOH-certified laboratory testing facility. (DOH-AO No. 0013 dated April
9, 2020)

Close Contact Listed would mean that close contacts and their contact details were identified and listed based on the
interview of COVID-19 case and/or as indicated in the COVID-19 CIF.
Close Contacts Traced are close contacts identified during the interview with a Confirmed COVID-19 Case,
and/or as indicated in the COVID-19 CIF with contact details.
● Close contacts are individuals who were exposed to a confirmed COVID-19 case 2 days before the case's onset of
illness (if case is asymptomatic, use date specimen was collected) until the case tests negative.
● They were individuals who had lived with, cared for, traveled with, or transacted with a confirmed case who had:
○ a) direct contact with a confirmed case, OR
○ b) face-to-face contact with a confirmed case for at least 15 minutes, OR
○ c) was with a confirmed case in an enclosed space for at least two hours.

Example: You have traced 6 confirmed cases and these yielded a total of 120 close contacts who were traced and
assessed. Divide 120 by 6 and your answer is 20. Thus, the case to close contact ratio is 1:20.
The sum of Columns L, M, N, and O is equal to Column F

These are for Confirmed Cases who are asymptomatic or with mild signs or symptoms. The data can be moved to
the following:
● Health Facilities/Referral Hospitals
● Died
● Recovered
The data source will be LHO, Local COVID-19 Task Force or any equivalent. Daily monitoring is to be conducted by
the facility manager or person in charge.

These are symptomatic confirmed cases confined at any referral facilities/hospitals The data can be moved to the

Case definition of a suspect case (as defined in DOH AO No. 2020-0013 dated April 9 2020, page 4, Item VI.A.2.1 a,
b, and c)
Suspect case - is a person who is presenting with any of the conditions below:
1. All SARI (Severe Acute Respiratory Infection) cases where NO other etiology fully explains the clinical
presentation;
1.1. ILI (Influenza-Like Illness) cases with any one of the following:
1.1.1. With no other etiology that fully explains the clinical presentation AND a history of travel to or residence in an
area that reported local transmission of COVID-19 disease during the 14 days prior to symptoms onset, OR;
1.1.2. With contact to a confirmed or probable case of COVID-19 in the two days prior to onset of illness of the
probable/confirmed COVID-19 case until the time the probable/confirmed COVID-19 case became negative on
repeat testing.
1.2. Individuals with fever or cough or shortness of breath or other respiratory signs or symptoms fulfilling any one of
the following conditions:
1.2.1. Aged 60 years and above;
1.2.2. With comorbidity;
1.2.3. Assessed as having a high-risk pregnancy; and
1.2.4. Health Worker.

who was laboratory-confirmed for COVID-19 in a test conducted at the national reference laboratory, a
subnational reference laboratory, and/or DOH-certified laboratory testing facility. (DOH-AO No. 0013 dated
April 9, 2020)
These shall refer to the management and outcome of close contacts traced and assessed, EXCLUDING the close
contacts subsequently classified as CONFIRMED CASES.
The sum of columns X, Y, Z, AA, and AB = The sum of P, Q, R, T, U, X, and W.
The data source will be the Local COVID-19 Task Force/DOH or equivalent.
The Joint AO 2020-0001 between DOH and DILG provides the guidelines for quarantine.
