1.0 INTRODUCTION

The preparation and submission of this report is a compliance of statutory


requirement of Occupational Safety and Health Act 1994 Occupational Safety and
Health (Safety and Health Officer) Regulations 1997.

2.0 SCOPE

This report will cover the Safety & Health Officers report content as stipulated under sub-
regulation (2) of REGULATION 19 of Occupational Safety and Health (Safety and
Health Officer) Regulations 1997.

3.0 PURPOSE

This report is prepared for the purpose to acknowledge the MICROSITE HAV SDN.
BHD. pertaining of Safety & Health activities, performance statistic, Safety & Health
preventative plan and actions taken along the month of FEBRUARY 2020.

4.0 SAFETY & HEALTH EXECUTIVE SUMMARY

i. To carried out Tool Box Briefing (TBB) to workers regard to HSE issue at site.
Some of the topics highlight during the toolbox briefing such as;

• Excavation work, Pipe Laying Work, Safe work procedure, Safety barricade and
signage at danger work place, Flow of traffic in the area (drive slowly), PPE,
Important of safety and health at workplace, Proper Housekeeping, Environment
issues, lifting activity, First Aid, Accident report, Assemble Point during
emergency, Heat stress, open burning issue, drive slowly at site & etc.
ii. To minimize the working hazard at work place, site walkabout and inspection by
Safety & Health personnel and supervisor is being carry out at site.
iii. To provide information to prevent health and safety risk at site.

iv. To assess site hazards and plan appropriate safe and healthy methods of working.

v. To assign to all relevant personnel their role and responsibilities for all other aspects
of site safety, health and welfare.

5.0 PROJECT DESCRIPTION

The project involves the construction and completion of DESIGN,


CONSTRUCTION, COMPLETION, TESTING AND COMMISSIONING OF
PROPOSED PACKAGE NR4 (NORTHERN REGION) FOR SARAWAK WATER
SUPPLY GRID PROGRAMME – STRESSED AREAS.

The Jabatan Bekalan Air Luar Bandar Sarawak had appointed MICROSITE
HAV SDN. BHD. to be the contractor to undertake the project.

6.0 WORK ACTIVITIES AT SITE

i. Excavation Works

- Excavation works is defined as removal of soils in order to build a building foundation


or to replace with suitable material for construction works. As a result, is open trenches
and the stability of the wall shall be affected by heavy rains, drying and improper slope
excavated.

Risk involved

• Collapse of side due to lack of or inadequate or weak shoring.

• Falling of spoil placed at edge of excavation.

• Water seepage.

• Vehicles being driven into the excavation due to driving errors, inadequate barriers.

• Collapse of adjacent structure due to disturbance to its foundations.

Control Measures

• All excavation works exceeding a depth of 1.5meters (5 feet) must be shored or sloped
to prevent people cave in.

• Vehicles, equipment and materials must not be positioned or driven near to excavation
area, as it may cause the excavation collapse or fall inside to the excavation area.

• All excavation area shall be cordon off using barricade to prevent persons or vehicle
from falling down.

• Where excavation points are located adjacent to the pedestrian or public road, warning
light must be providing during nighttime.

• Excavation works shall be supervised by competent person or designated person.

• During back filling is carried out by bumper, front loader or excavator, all person must
be out of working area or in safe location.

• Proper signage shall be installed to alert public from excavation area.

ii. Pipe Laying Work

- Pipe laying work is referring to pipe installation to transport natural resources from the
place of extraction to where they can be used or even within the place of their
extraction.

Risk involved

• Pipes may move/roll or be tampered with by others.

• Collapse of trench.

• Slip, trip and falls.

• Falling of spoil placed at edge of excavation.

• Electrocution if using power tools in water-charge environment.

• Vehicles being driven into the excavation due to driving errors, inadequate barriers.

• Failure of existing pipeline under pressure.

Control Measures

• Provide secure stockpile area for pipes and fittings.

• Minimize height of stockpile.

• String only sufficient pipes for day’s work.

• All excavation area shall be cordon off using barricade to prevent persons or vehicle
from falling down.

• Where excavation points are located adjacent to the pedestrian or public road, warning
light must be providing during night time.

• Proper signage shall be installed to alert public from excavation area.

• Call and check relevant Authority (e.g. power, water, gas, council) record for location
of services.

• Ensure adjacent stop valves are closed / operational.

• Relieve pressure in system.

• Use only skill/competent person to perform the task.

iii. Lifting Works

- Building construction require heavy lifting works to carry out heavy construction
materials from one place to another. Heavy lifting work requires heavy machinery such
as mobile crane.

Risk involved

• Poor signaling may lead to accident.

• Overloading due to lack of ability to estimate the load.

• Over speed, swinging and apply brake abruptly will cause the jib to snap or caused
machine overturn.

• Used of damaged sling rope.

Control Measures and safe working procedures

• To ensure crane has valid approval permit from DOSH (PMA).

• To ensure crane are operated by competent person and possess competency certificate
from DOSH or CIDB.

• Erection and dismantling crane to be done by competent person.

• To ensure crane are regularly checked by competent mechanic.

• To appointed competent signalman to assist crane operator.

• To barricade the area of heavy lifting works.

• To ensure adequate signage for lifting work.

iv. Pilling Driving

- The civil work and building construction require piling work to be carried out.

Risk involved

• Lifting hazard.

• Ergonomics.

• Moving plant.

• Crushing hazards.

• Overhead & underground services.

Control Measure
• Piling operations shall be undertaken or carried out by experience subcontractor, with
the assistance of competent and qualified personnel.

• Where there is any question of stability of structures adjoining areas to be piled, such
structures shall be supported where necessary by underpinning, sheet piling, shoring,
bracing or other means in accordance with the design of a Professional Engineer to
prevent injury to any person.

• All pile-driving equipment shall be inspected daily by a designated person before work
starts and all machinery defects shall be immediately rectified or make good before pile
driving commences.

• Every piling frame and its components shall be thoroughly examined by an approved
person at least once in every twelve months but can be more depending on the condition
of the components.

• Reasonably practicable measures shall be taken to warn workers not to approach within
50 meters of pile under test.

• Before placing or advancing a pile driver, a designated person shall inspect the ground
and where necessary for firm and level footing, timber shall be placed. After placing or
advancing a pile driver, inspection and improvement of footing shall be made, when
necessary to maintain stability.

• All mechanical equipment and plant shall be maintained throughout the piling work.

• Piling materials need to be arranged and stockpile at specified place and not to endanger
to traffic and public.

Control Measures

• Persons selected to drive site vehicles should be physically fit with normal eyesight and
hearing.

• The main access and egress point into and from site should be appropriately planned.
Roads on must be regularly maintained and should be free of potholes and uneven
surfaces.

• Planned maintenance procedures prevent accident and delays due to mechanical


failures, minimize repair downtime and prevent excessive wear and breakdown.

• Speed limit should be established early and clearly displayed for traveling on site.

vi. Working at Height

- Working at height describes work undertaken off the ground. Commonly it involves the
use of scaffolds, ladders, hoists, gantries and general roof work. Work on roof is
particularly dangerous. The level of control will depend on whether the roof has edge
protection or not allied to the plant or roof lights that may be upon it.

• Slip and fall

• Fall from height

• Fall protection failure

• Unprotected edges

• Dropped object from height

Control Measures

• Ensure Method Statement covering the following – access and egress procedures,
fall prevention, supervision levels and work equipment that may be used.

• Access to roof must be restricted so doors leading to it must be kept locked and key
are kept only by Work Supervisors for the area.

• For unguarded roofs a safety barrier away from the edge should be used to identify
the work areas and safe access routes.

• Walkways across the roof should have a barrier each side. Where plant or
equipment is within 2 meters of an unprotected edge a barrier must be erected to
prevent falls.

• Alternatively, a fall arrest device may be used.

• Roofs constructed of fragile materials must be accessed only by use of roof ladders
or crawling boards coupled with a fall arrest device.

• Roof lights are a particular hazard, as they are not loading bearing. These should be
clearly marked or painted and be protected by barrier where possible.

• Debris must never be thrown from height. Chutes or baskets must be used.

• Carried out proper housekeeping to prevent dropped object from height.

7.0 CONSTRUCTION INDUSTRY DEVELOPMENT BOARD (CIDB)

- Any workers without CIDB green card will be arranged to attend Safety Induction
Construction Worker (SICW) training accordingly.
