INSTRUCTIONS

1. Make sure the subsequent instructions are carefully read and understood. Everything must be
clear to the raters before filling out this form. Ask assistance when necessary.
2. This evaluation sheet shall be duly accomplished by the intended employee himself (RATING:
Column A) and by his/her peer-rater (RATING: Column B). Leave Column C empty as this will be
used for validation purposes.
3. First to rate is the employee himself in RATING A. After completion, the employee shall submit and
forward this to the HR Manager to facilitate the next procedure/s.
4. Rater/s must be fair and honest enough in supplying necessary ratings. All responses are subject
to thorough validation. DO NOT LEAVE AN ITEM BLANK.
5. Supply essential ratings demonstrating individual level of performances parallel to the key
performance indicator provided: Part I -Specific Criteria (Based on Performance); and Part II:
General Criteria (Based on Crucial Components)
6. Raters need to affix their signatures as a way of certifying responses to be true and correct
signifying validity and reliability.
7. Responses will be treated with maximum confidentiality. All parties are required to exercise
utmost secrecy. Reproducing and/or posting copies in any form is strictly prohibited.
8. Comments, suggestions and recommendations are encouraged and will be noted accordingly.
9. This appraisal procedure aims to measure effectiveness and efficiency towards the assigned
work/s. The need to enhance and promote programs for personnel’s development lies in the
raters’ authenticity.

involves meeting and constant communication with project engineer to
clearly understand the needs and budget, which is required to provide
effective supervision.
5 Facilitates the purchase and delivery of resources - coordinates with the
supply chain management team to order the resources necessary to
complete a specific project hence, responsible for the timely delivery of
these resources.
6 Makes sure projects are completed on time and on budget - performs all
the tasks necessary to keep a project on time and on budget involving
the tracking of financial records, shipment records, daily labor logs, as
well as constant communication with the project manager.
