COLLEGE OF NURSING ENROLLMENT PROCESS (BSN First Year)

STEP 1. Print the CON Reference Code and affix your full name and signature below the code.
For verification purposes send a scanned copy together with documents in Step 2.

STEP 2. Submit scanned copy of the following documents as previously required by the OASP
via email to nursing@cmu.edu.ph
A. Confirmation slip from the OASP
B. Notice of CMUCAT Result
C. Form 138 ( Senior High School Report Card)
D. Student Personal Date Sheet (CMU-F-1-ACA-040). To be encoded or printed with
photo and signature.

STEP 3. Answer the written interview questions of the College of Nursing Screening Sheet (CMU-
F-1-CON-003). Please email back the answered form with the functional mobile
number and messenger account.

STEP 4. The CON screening committee will contact you for a video or phone interview anytime
within Monday to Friday 8:00 am to 5:00 pm timeline.

You will be notified of the screening results.

For those who passed the screening process, will need to comply the following:

STEP 5. Download the following forms:

A. Student Health Assessment Record (CMU-F-1-CON-004).

B. Dental and Medical Forms 1 2 and 3. (downloadable at doorstep@cmu.edu.ph)

STEP 6. Secure Laboratory/Diagnostic Test Results to credible laboratories near your respective
areas following Community protocols.
A. Chest X-ray
B. Drug test
C. Complete Blood Count (CBC)
D. Urinalysis
E. Stool Exam
F. Pregnancy Test
G. Hepatitis B Ag Test (HBsAG)

STEP 7. Bring all documents from Step 5 and Step 6 for physical and dental examination by a
government physician nearest to your area. Make sure that all laboratory/diagnostic test
results are evaluated and the Student Health Assessment Record and Dental and Medical
Forms are accomplished and duly signed by the examining physician.

STEP 8. Send scanned copies of accomplished documents in Steps 5, 6 and 7 via email to
nursing@cmu.edu.ph
