INFORMED ASSENT FORM FOR PARTICIPANTS

This informed assent is for the students enrolled in Grade 11 Senior High
School. They will be invited to participate in a study entitled “The Impact of Social
Influence to the Grade 11 Senior High School Students’ Preference in buying
product.

This Informed Assent Form has two parts:

1. Information Sheet ( gives you information about the study).
2. Certificate of Assent ( this is where you sign if you agree to
participate).
You will be given a copy of the full Informed Consent Form.

Part I: Information Sheet

Introduction
Good day! We are the Grade 11 Senior High School students of Gabriel
Taborin College of Davao Foundation, Inc. We are doing our research entitled:
“The Impact of Social Influence to the Grade 11 Senior High Students’
Preference in buying product” we are going to give you information and invite you
to be part of our research. But if you wish to take part of our research, you do not
have to.
You may discuss anything in this form with your parents or friends or to
anyone else you feel comfortable talking to. You can decide whether to
participate or not after you have talked it over. You do not have to decide
immediately.
This assent form may contain words that you do not understand, please
ask us to stop as we go through the information and we will take time to explain.
If you have questions, later you can ask us about it.
Purpose
The purpose of this study is to survey and find out the impact of social
influence to the grade 11 senior high school students’ preference in buying
product.
Participant Selection
The participants of this study are the Grade 11students of GTCD. You
have been chosen through random sampling.
Voluntary Participants
Your approval for participation in this research is entirely voluntary. It is
your choice whether you choose to agree or not, you will not be forced.
We have checked with the students and they understand that participation
is voluntary.

Procedures
There will be 49 participants in this study which are chosen through
random sampling from your year level. We are asking you to help us learn more
about the impact of social influence to the grade 11 senior high school students’
preference in buying product. We are inviting you to participate in this research. If
you are agree, you will be asked to answer the survey questionnaire that would
take at least 3 minutes.
All participants will be asked to participate in the survey method. During
the survey, one of us will sit down, with you in a comfortable place. We will wait
for you to finish answering the survey questionnaire. You may also ask questions
to us if you are continue with our questionnaire.

Risks

I will ask you some questions that may be sensitive or personal. If you
would fell uncomfortable answering or taking about some of the topics, you do
not have to give me any reason for not answering or responding to any question
or for refusing to take part in the interview.

Reimbursement
 There will be no reimbursement of cost.

Confidentiality
This information that we collect from this research will be kept private. Any
information about you will be coded instead of writing it name. Only we will know
the number correspond to you and we will lock the information up with a lock and
key. It will not be shared with or given to anyone.

Sharing the Result

Nothing that you tell us will be shared to anybody outside the research
team and nothing will be attributed to you by name. However, the result will be
published in my manuscript or in journal so that other interested people may
learn from the research.

Right to Refuse or Withdraw

You have the right to refuse or withdraw from participating in this research
without the need to explain your reasons.

If you have any questions, you may ask Mr. Rhyan Mike R. Bacaro, a
GTCD Instructor and our research adviser.
If you choose to be part of this research, we will also give you a copy
of this paper to keep for yourself. You can ask your parents to look after it
if you want.
