HEALTH PROBLEMS Nursing Diagnosis
1. Threat to cross infection (cough and colds) 1.Inability to recognize the presence of the
condition or problem r/t attitude/ philosophy in
life which hinders recognition of a problem as
manifested by verbalization of Mr.Hubert
Aquinalde that his condition is just colds.

2. Presence of breeding place 2.Inability to provide a home environment

conducive to health maintenance r/t presence of
breeding place in the clients’ house as manifested
by a drum of stagnant water outside the clients’
house.

3. Air pollution 3.Inability to provide a home environment

conducive to health maintenance r/t rampant air
pollution in clients’ surroundings as manifested by
the location of clients’ house along the highway
that many vehicles pass by everyday.

4. Smoking 4.Inability to make decisions with respect to

taking appropriate health action r/t failure to
comprehend the nature/magnitude of problem or
condition as manifested by Mr. Albert Aguinalde
smoking 6-7 or more sticks a day.

5. Lack of immunization 5.Inability to recognize the importance of

immunization r/t lack of time as manifested by
member’s preoccupation with own concerns and
interest.
Px #1: Threat to cross infection( cough and colds)

Criteria Score Justification Computation Scale

A.Nature 3 Actual problem in health maintenance that usually needs 3/3 x 1 1

immediate attention in order to prevent complications
B.Modifiability 2 Highly modifiable because all resouces are available such 2/2 x 2 2
as medicine and interventions to manage the problem with
the presence of HW in the family and a near clinic in the
community.

C.Preventive 2.25 Moderate Preventive potential because one is 2.25/3 x 1 0.75

Potential comprimised;duration of the problem
D.Salience 2 High Salience because a,b ,and c are met. 2/2 x 1 1

      TOTAL 4.75

P#2 Smoking
Criteria Score Justification Computation Scale

A.Nature 2 Conditions conducive to the development of further 2/3 x 1 0.67

diseases such as tuberculosis, lung cancer, &other
respiratory infection
B.Modifiability 2 Highly modifiable because all resouces are available such 2/2 x 2 2
as medicine and interventions to manage the problem with
the presence of HW in the family and a near clinic in the
community.
C.Preventive 1.5 Low preventive potential because of high risk in family 1.50/3 x 1 0.5
Potential member but has beeen doing appropriate management
like lessening sticks per day(6-7sticks/day(before),2-
3sticks/day(now),and smoking outside the house
D.Salience 2 High Salience because a,b ,and c are met. 2/2 x 1 1
      TOTAL 4.17

P#3:Presence of Breeding Place

Criteria Score Justification Computation Scale

A.Nature 2 Conditions conducive to the development of disease that 2/3 x 1 0.67

can be postponed.
B.Modifiability 2 Highly modifiable because all resouces are available such 2/2 x 2 2
as;they have the money to improve their drainage,
interventions to manage the problem with the presence of
HW in the family and a near clinic in the community.
C.Preventive 2.25 Moderate preventive potential because the problem is not 2.25/3 x 1 0.75
Potential yet resolved, they all have the resources but they give less
importance to the problem
D.Salience 1 Low Salience because the family verbalized it as a problem 1/2 x 1 0.5
but they think that it can be postponed and have future
plans to resolve the problem.
      TOTAL 3.92
P#4:AirPollution
Criteria Score Justification Computation Scale

A.Nature 2 Conditions conducive to the development of disease that 2/3 x 1 0.67

does not require immediate attention
B.Modifiability 2 Highly modifiable because all resouces are available such 2/2 x 2 2
as;they have the current knowledge, technology,
interventions to manage the problem with the presence of
HW in the family and a near clinic in the community.
C.Preventive 0.75 Low preventive potential. The family made an appropriate .75/3 x 1 0.25
Potential current mangement by closing the window of their house
near the street but they compromised the gravity, duration
of the problem, and exposure to a vulnerable/risk family
member.
D.Salience 1 Low Salience because the family verbalized it as a problem 1/2 x 1 0.5
but they think that it can be postponed.
      TOTAL 3.42

P#5: lack of immunization

Criteria Score Justification Computation Scale

A.Nature 1 Foreseeable crisis due to anticipated periods of usual 1/3 x 1 0.33

demand on family on individual & family in terms of
resources.
B.Modifiability 1.5 Partially modifiable because 1 resouce is unavailable: the 1.5/2 x 2 1.5
current knowledge, technology, interventions to manage
the problem with the presence of HW in the family and a
near clinic in the community.
C.Preventive 0 Low preventive potential because all are compromised. 0/3 x 1 0
Potential
D.Salience 0 0-Salience because all are not identified, recognized nor 0/2 x 1 0
accept that the problem exists.
