The World Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic.

Due to its capacity to transmit from person-to-person through respiratory droplets, the government has set
recommendations, guidelines, and some prohibitions which Golden Road Brewing and Wags and Walks
adhere to and with which participants in the Hazy Pup Dog Delivery program must comply.

In consideration of my participation in the Hazy Pup Dog Delivery program, you acknowledge and agree to
the following:
❏ I am aware of the existence of the risk of my participation in the Dog Delivery program that may
cause injury or illness such as, but not limited to, COVID-19.
❏ I have not experienced symptoms of fever, fatigue, difficulty in breathing, dry cough, or exhibited
any other symptom related to COVID-19 or any other communicable disease within the last 14
days.
❏ I have not, nor any member(s) or my household, traveled by sea or air internationally within the last
30 days.
❏ I have not, nor any member(s) or my household, diagnosed with the COVID-19 virus in the last 30
days.

Following the above, I hereby declare the following:

❏ I am fully and personally responsible for my own safety and actions while and during my
participation.
❏ I understand that if I do not wear a mask or other facial covering during the Dog Delivery program,
that I may be denied participation.
❏ I agree to utilize hand sanitizer prior to any contact with dogs and after any contact with dogs.
❏ I agree that no more than four individual people from my household participate in the Dog Delivery
program.
❏ I agree to maintain social distancing precautions (6ft) between myself and staff from both Wags and
Walks and Golden Road Brewing.

By signing below, you also acknowledge that by participating in the Hazy Pup Dog Delivery program, you
expressly waive any and all rights, claims, and causes of action in perpetuity, including, without limitation,
any Claim involving any injury (including, without limitation, bodily injury or property damage) suffered
while you are engaged in the Hazy Pup Dog Delivery program.

By signing below, you also acknowledge that your appearance in promotions (including, without limitation
print media, social media, online or otherwise) for and/or on behalf of Wags and Walks and Golden Road
Brewing constitutes part of your participation in the Dog Delivery program and that you shall not be
entitled to any compensation for such appearances. Furthermore you hereby release Wags and Walks and
Golden Road Brewing, its employees, volunteers, directors, officers and affiliated parties from any and all
claims and demands arising out of, or in connection with such use.
