Hazard ID and risk management
We have identified the hazards that are risks to health and safety for the work we are doing and these are listed in the next section. We have also identified the appropriate risk controls
required to eliminate or minimise the risks to health and safety. These controls are shown in the next section and will be put in place prior to work starting.

The hazards and risk controls for this work will be discussed with workers and all visitors as part of their induction to the work site. Workers will be made aware of their responsibilities to
ensure risk controls are in place and effective, or report to their supervisor any risks to health and safety that are not being adequately controlled.

Any risk to health and safety caused by ‘high risk’ hazards (such as working at heights) will be identified and controlled (eliminated or minimised) using a Task Analysis process.

This hazard ID and risk management process is documented in our Health and Safety Procedures. All current hazards on the work site will be listed on the site Hazard Board.

The hierarchy of control


Hazards are to be assessed and controlled by eliminating or minimising any risks to health and safety.

Eliminate - Where the risk to health and safety is removed from the workplace.

Minimise - Where the risk cannot be eliminated it should be reduced by one or more of the following; substitution, isolation, and implementing engineering controls (guarding, barriers etc). If
the risk still remains it can be further minimised using administration controls (procedures, documentation etc) and PPE. Where possible use multiple controls. All risk controls are ranked in
order of most effective (eliminate) to least effective (PPE) using the following key:

Eliminate (avoiding the sources of harm) - ELI

Substitute (using a less hazardous thing, substance or work practice) - SUB*

Isolate (separating people from the hazard) - ISO

Engineering (using physical control measures) - ENG

Administration (using safe methods of work, processes or procedures) - ADM*

Personal Protective Equipment (using safety equipment to protect against harm) - PPE

This section lists each known hazard, the associated risks and the good practice controls to manage these risks. Where possible we will eliminate the risk to health and safety, if we can’t
eliminate the risk then we will put in place controls to minimise by substitution, isolation, or implementing engineering controls. If the risk still remains it will be further minimised using
administration controls and PPE. At all times we will implement the following controls:

Safety training
All workers on this job are either competent (have been formally trained, are experienced and have demonstrated their competence) or are supervised by a competent person.

All workers who undertake specific high-risk work on this job have received formal training in the appropriate safe working practices.

We have a documented method to conduct regular safety training in our Health and Safety Procedures document.

All training for our workers has been recorded on our attached Training Register.

Personal Protection Equipment (PPE)


All workers on this job are required to wear/use the following PPE, during their work on our work site. Training in its care, maintenance and use will be provided. The PPE required on this

Event reporting procedures
All events (injury/illness/incident/near miss) will be reported to the Supervisor, Project Manager and/or the Main Contractor as soon as possible. All events will also be reported to HazardCo
who will provide assistance to report and record the event. All events will be recorded in our Event Register.

If an event occurs, the priority is to provide first aid (if applicable) to any injured person, then to preserve the scene. Under no circumstances will the scene be cleaned up or interfered with
until the Supervisor has consulted with HazardCo or a WorkSafe NZ Inspector who has released the scene.

Once identified all Notifiable Events will be reported to WorkSafe NZ by the fastest means possible and in writing if requested using the prescribed form.

All events will be documented using our Event Register.

Job and visitor induction


All workers will be inducted to the work site by scanning the QR code on the Hazard Board, if the worker does not have a QR scanner then they can go to www.hazardco.com/site-induction
and complete the online induction.

As part of their induction, workers will acknowledge they have read and understood the content of this SSSP and any relevant Task Analysis (TA) docments.

Inductions will be recorded online or in our Induction Register which will remain in the work site for the duration of the job.

All clients and general visitors will either be inducted (hazards/risks explained) or escorted whilst they are on our work site.

Site reviews
Site Reviews will be conducted and recorded monthly using the HazardCo App or Site Review Checklist.

Toolbox meetings
Toolbox Meetings will be conducted and recorded using the HazardCo App or Toolbox Meeting Checklist.

All workers that are present on the work site during a toolbox meeting are expected to attend and are encouraged to provide input on matters that affect safety for this job. All attendees will
be captured through a photo in the HazardCo App or listed on Toolbox Meeting Checklist as a record of attendance.

Sub-Contractors
Each of our sub-contractors (PCBUs) have been pre qualified to ascertain that they have their own active health and safety management system containing health and safety tools and safety
instructions.

The competence of our sub-contractors has been assessed or reviewed eg they have the appropriate qualifications and skills for the job they are undertaking.

All sub-contractors will be inducted to the work site as outlined in our induction procedures.

The sub-contractors for this job are listed on our Contract Worker List.

Plant, machinery and equipment


All plant, machinery and equipment on site will be in good working order, with all safety mechanisms intact.

Where applicable, machinery will be operated and maintained in accordance with manufacturer's instructions or Standard Operating Procedures.

If any equipment is not in safe working order, it will be removed from the work site for servicing and will be repaired by a competent person before use.

All plant, machinery and equipment will be recorded on our register and checked regularly.
