AFFIDAVIT OF EXPLANATION

I, GRACE UBUNGEN CAMERO of legal age, Filipino citizen, single and a resident of Brgy.
Bulanos, Narvacan, Ilocos Sur, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and state:

1. That I applied for a passport with DFA Baguio last August 16, 2014 as a new
applicant;

2. That while I was waiting for the release of my passport, DFA texted me saying that
the release of my passport is on hold due to an undeclared previous passport
issuance. They told me to return to their office for further interview;

3. That I was indeed issued a passport with number FF525307 on 23 March 2000 by
DFA La Union. That due to transfer of residence, said passort was included with
other documents and belongings. But I was not able to monitor where it was placed;

4. That at the time that I filed for my passport application with DFA Baguio, my old
passport was not on hand and I thought that it would be easier for me to apply as a
new applicant instead;

5. That on the same day the DFA texted me, I immediately looked for my passport in all
our travelling bags, old papers and clothes that was kept in our old wooden cabinet
which was already partially ruined by previous typhoons;

6. That I found out my passport which was already damaged and has stains and almost
on all of its pages;

7. That I am surrendering my old passport for cancellation;

8. That I undertake not to commit the same / similar mistake in the future.

IN WITNESS WHEREOF, I have hereunto set my hand this 4th day of October 2014, in
