Presenting Complaints:
The participant has the feeling of headache and anxiety at some time when he had over wok of
school and other work pressure.He has fever most of the time and also have
Developmental History of problem:
The participant has the deficiency of glucose at sever level with high intensity. He admitted in
Hospital due to glucose deficiency .The duration of this deficiency was 3 to 4 years.

Due to glucose deficiency the participant has faced different medical psychological and social
incidents like due to deficiency of glucose participant has fever most of the time.His social life
circle is also disturbed cause of glucose and his personality is becoming confused and introverted
and due to his illness.

Background Information:

Developmental History:
The patient born with normal delivery and he faced many ups and down in his life.He faced such
disease most of the time in his life .He has the deficiency of glucose and due to this he admitted
in the hospital several times.the deficiency of glucose is covered by articificial glucose like drip.

He cried according to the normal children but he started crawling late according to normal time
period. He had difficulty in walking and he started walking late in his life

Personal History:
The participant has normal daily routine schedule he awake at 5 in the morning mostly and after
saying prayer he goes to school at the time his leisure activities are reading books and he mostly
avoid such activities that required movement. Mostly he likes go to school and he has great
interest in study.

Family History:
The participant has the macular family system and he has five family members include 3 brother
and sisters including the participant and his parents including mother and Father. He has friendly
environment in his family his parents can discuss any issue and he has the friendly relationship
with other family members. His parents are well qualified his father is school teacher and his
mother is also well qualified and she is house wife

Educational History:
He started school education late according to averages. He started school at 6 year of age. His
school performance is very good he mostly secures position in annual exams. He has not good
interaction with his teachers and with his friends he has limited friends. He also has limited and
low interaction with his class fellow. He has great interest in mathematics and his favorite game
is ludoo

Participant has limited friends and has introverted personality he never go to outing with friends.
