PARENTAL CONTRACT

Part 1 FINANCIAL RESPONSIBILITIES

The conditions of this agreement provide protection for our parents, as well as our school. In order
to assure that we can provide these services, it is essential for the school to be financially stable. Salaries
and overhead expenses cannot be reduced because of absentee losses. This contract is a commitment that
you will financially support the enrolled slot for your child.

I. A non-refundable registration fee of Php 500.00 per child will be collected upon enrollment to
provide him a slot in his class;
II. Miscellaneous Fee (Php4000) is required during enrollment period to be utilized in the
classroom improvement and procurement of teaching materials; Staggered payment can be
made until the end of the first month of classes;
III. Payment of workbooks must be given before its release.
IV. Monthly Tuition Fee (Php 1250) is collected every 18th of the month;
V. Other contributions for a particular school activity or project may be collected as approved by
the PTCA;
VI. Payments will be made at the School Office in cash;

VII. Refunds will not be made for any holidays or absent days (including trips and illness);
VIII. Clearing of School accounts will be done a week prior to scheduled Periodical Test and at the
end of every Semester. Failure to meet your financial commitment may result in withholding
the child’s permit to take the scheduled Periodical Test and the termination of services at the
end of the semester;

Part 2 WITHDRAWAL PROCEDURES

I. A signed, written or personal notice of permanent withdrawal by the parent or guardian must
be given 3 days in advance;
II. If withdrawal is made before the first day of class, Miscellaneous Fee and Monthly fees will be
refunded. Otherwise, the Monthly fee will be collected in full until the last month attended
and the miscellaneous fee will be forfeited. If withdrawal is made during the second half of
the school year, the remaining amount of the Tuition Fee for the whole school year must be
paid in full;
III. In the event of withdrawal, any standing school account must be paid before the Pupil’s
School Records be released.
IV. The provision of our service is conditional upon compliance of both the parents and child to
our Code of Behavior. Behavior of a child that poses a safety hazard to school properties and
other children may result in immediate withdrawal.

Part 3 CODE OF BEHAVIOR

The safety of all children is our primary concern. The following expectations are necessary to
promote a happy, safe and healthy atmosphere. The child and the parents shall at all times:

 Respect the building and equipment as well as the personal property of others.
 Show personal respect for all individuals through behaviour and language.
 Resolve conflict in a peaceful manner.
 Conduct them in a manner which allows others to feel safe from verbal and physical abuse.
 Avoid profane language, violent acts and indulgence in activities like smoking, gambling and
drinking hard liquor inside and few meters away from the school premises.

Part 4 REMOTE-LEARNING RESPONSIBILITIES

I. I understand that my role as a parent in the conduct of the Remote-Learning Modality includes
the following:
1. Prepare the workstation/ study area and the learning materials.
2. Assist my child in the technical procedures of remote learning.
3. Monitor that the child is participating in the online class as well as in the video lessons.
4. Encourage the child to study and do the offline activities or homework.
5. Submit the workbook every Friday for checking of Homework.
6. Make sure the child can take the assessment procedures.
7. Impose discipline.

II. I will help my child follow the rules in the conduct of the remote-learning modality.
III. I will cooperate with the teacher and maintain a strong partnership with the school in the
conduct of the Remote-Learning Modality.
IV. Should there be problems that concern me as a parent or the school, I will discuss it with the
teachers or the school administrator openly and honestly. I will not resort to public media such
as radio, print, social network, etc.

Part 5 PRIVACY INFORMATION

I hereby consent to the collection, use and disclosure of my parental and my child(ren)’s personal
information by the school administration for the purposes of providing child care services to my child(ren)
enrolled in the school. I understand that the school protects the privacy of all personal information in its
possession in compliance with prevailing privacy legislation and in accordance with the school’s privacy
policy.

Part 6 INTELLECTUAL PROPERTY RIGHTS OF INSTRUCTIONAL MATERIALS

I understand that the video lessons used for instructional purposes are created, published and
owned by the school. I hereby guarantee to abide by the provisions of the Intellectual Property Act such as
but not limited to: downloading and saving of materials other than that authorized by the school,
publishing of materials without prior written consent of the administrator, distribution of materials.

Should any instance or occurrence of violation of the Intellectual Property Act would come to my
knowledge, I shall report the same to the school administration.
