1) Collapse of G+3 storey building in
Khed
• The building was constructed when the
collapse occurred.
Reasons of failure-:
• Proper concreting practices were not
performed on site.
• proper sieve analysis was not done for design
mix which ultimately resulted into poor
concrete.
• No use of vibrators on site at the time of
construction of building resulting into poor
column and beam joints.
• lack of proper inspection.
• Poor quality workmanship.
2) Failure of Overhead Water Tank in
the State of Uttar Pradesh
• The state government of Uttar Pradesh had launched the safe
drinking water project for rural population living in remote
areas. The job of implementation of the scheme was given to
the corporation “JAL NIGAM” which deals with the public
water management in the state. However the project was a
failure.
• At last the problem was referred to the department of civil
engineering for the remedial measures. After minute study of
the problem, it was observed that various phases of project life
cycle were not tested on the test stone of money, man,
material, machine, maintenance and lastly wrong selection of
technology for constructing the tank.
Reasons of Failure

• The project launched had several shortcomings in the planning


phase.
• Location of site of project.
• Poor quality workmanship.
• Technology used for construction of water tank.
• Improper selection of material, mix, placement, compaction,
leaking formwork temperature control, curing leading to
honeycombed and porous concrete.
• Creating funds for the maintenance.
3) 2000 Commonwealth Avenue-
Boston
• The structural failure and eventual collapse of the 2000
Commonwealth Avenue building occurred January, 25 1971.
Failure occurred in three stages beginning with punching shear
in the uppermost floor of the 16 story building. This was
followed by the collapse of the roof slab and the eventual
progressive collapse of the east side of the building. Four
workers were killed and 20 injured as a result of this failure .
• Immediately after the collapse occurred, rescue efforts were
delayed due to uncertainty of structural stability for the
remaining building as well as other concerns for rescue
workers safety .
Reasons of Failure
• Almost all jobs were sub contracted
• No architectural and engineering inspection done
• Inadequate inspection by the city of Boston
• The general contractors representative was not a licensed builder
• Construction was based on arrangements done by the
subcontractors
• No direct supervision of subcontractors.
• Not following the structural engineers specifications for
shoring and formwork
• Lack of concrete design strength
• Lack of shoring or removed too soon
• Improper placement of reinforcement
• Little construction control on site
• Owner changed hands many times
