TO WHOM IT MAY CONCERN:

It is my pleasure and honor to provide a letter of professional reference to MS. LEMEE RUTH
GRANADA PAAT who was one of my valued staff nurses in the Nursing Department for 9
months in Sta. Rosa Hospital and Medical Center, a tertiary hospital with 99-bed capacity. She
worked as a Neonatal Intensive Care Unit Staff nurse from June 28, 2016 to March 22, 2017.
She has worked with us as a Full Time Registered Nurse, and rendered 48 hours per week,
working in different shifts (morning, evening and night shifts) even overtime whenever the unit
required her to do so.

Under the ANZSCO code, 254415 Registered Nurse (Critical Care) our Neonatal Intensive Care
Unit falls under that category since it deals with Acute Critical Patient Care. This unit has a 13-
bed capacity that offers intensive care services 24 hours a day, 7 days a week. It has state of the
art equipment and caters various specialties mostly on Premature, Cardio, Infectious and
Surgery.

As a staff nurse, she provides professional nursing care and coordinates care planning with
other disciplines. She provides care that meets the psychosocial, physical and general aspects of
care and determines priorities of care based on physical and psychosocial needs of patients, as
well as factors influencing patient flow through the system. She communicates with
Neonatologists and other Specialty Physicians regarding changes in the patient’s status,
symptomatology and results of diagnostic studies and responds quickly and accurately to
changes in condition or response to treatment.

The duties and responsibilities of a Neonatal Intensive Care Unit Staff Nurse in SRHMC are as
mentioned below and in which MS. PAAT has achieved in her length of stay in the unit.

KEY ACHIEVEMENTS:
1. OrganizingPatient Care
1.1 Performs head-to-toe assessment on all patients.
1.2 Checks vital signs, monitors blood pressure, temperature and heart rate. Monitors the
hemodynamic status of patient and correctly interprets the results, institutes remedial action
when appropriate.
1.3 Determines knowledge in manipulating equipment such as incubator, phototherapy

1.4 Provides direct patient care, evaluates outcomes, consults with team workers as required and
adjusts nursing care processes as indicated to ensure optimal patient care.
1.5 Administers, chart medications and patient’s reaction to such treatments. Follows the ten
medication rights and reduces the potential for medication errors.
1.6 Prepares, hangs, monitors and discontinues intravenous (IV) fluids including intravenous
complex medications such as inotropic drugs.
1.7 Provides body hygiene, assists in moving/turning, maintaining proper alignment
utilizing proper body mechanics.
1.8 Observes proper care for contraptions such as peripheral and central intravenous lines,
endotracheal tubes, Orogastric tubes and catheter
1.9 Showcases distinct knowledge and skills in caring for patients with mechanical ventilator
assisted breathing.
1.10 Identifies and addresses psychosocial, cultural, ethnic and religious or spiritual needs of
patients and their families and involves the patient and family in the formation of the plan of
care.
1.11 Formulates a teaching plan based on identified learning needs and evaluates effectiveness
of learning for the family member involved in patient’s care.
1.12 Maintains confidentiality of all patient information.

2. Management of Resources and Documentation


2.1 Maintains accurate and continued nursing documentation including patient histories,
conditions, treatments, responses and assessment changes.
2.2 Assists in cost containment through the appropriate ordering and conserving of supplies and
equipment.

3. Collaboration and Teamwork


3.1 Assists with minor surgical procedures, supplying physicians with needed surgical tools and
instruments.
3.2 Assists or supervises in the orientation of new assignees to the Department.
3.3 Demonstrate the ability to assist physicians with procedures in the neonatal intensive care
unit such as surfactant administration or endotracheal tube insertion.
3.4 Cooperates with other personnel to achieve departmental objectives; maintains good
employee relations and the interdepartmental objectives.
4.6 Performs such other related duties and responsibilities as may be assigned or requested
by immediate superior from time to time.

5. Observance of Policies, Standards and Professional Development


5.1 Maintains established departmental policies and procedures, objectives, performance
improvement progams, safety, environmental and infection control standards.
5.2 Maintains current knowledge of medications and their correct administration based on age
of the patients and their clinical condition.
5.3 Enhances professional growth and development though participation in educational
programs, current literature, in-service meetings and workshops; shows responsibility for
own professional practice and ongoing education.
