This form should be used to submit a request to the weekly Establishment Control &
Training Panel for consideration of;
 Individual requests for CPD funding for training & development activities (subject to
repayment provisions in line with the Training & Development Policy).
 Manager requests for training & development activities to be funded from the central
training budget.

Completed forms should be submitted to: Sandra.kadungure@nhs.net by COP each

from my final salary.
