ACTIVITY TITLE: CULMINATINATING ACTIVITY AND RECOGNITION DAY OF

STA. MARCELA NATIONAL HIGH SCHOOL FOR THE FIRST


QUARTER OF THE SCHOOL YEAR 2018 – 2019

PROGRAM TERMINAL REPORT (PTR)

SUMMARY OF ACCOMPLISHMENT:

Background and Rationale:


Culminating activity is an integrative part of the curriculum
aimed at presenting accumulated knowledge made by the students at a
certain quarter of the year. Activities may be in the form of
researches, portfolio, OJTS exhibit, or performances.
SMNHS, through the initiative of the Schools Division
Superintendent, headed by the Principal, conducted a simple
culminating activity using socio – cultural activities as the main
theme.
To be able to maximize the activity, recognition program and card
giving day were also included so that parents would be able to witness
the presentation of the said performances.
Hence, this activity.

Program Objectives:
Through the conceived activity, the following objectives were
then determined:
1. To present the accumulated and integrated knowledge and learnings
of the students in the different subjects through socio –
cultural activities; and
2. To recognize academic and extra – curricular achievement of the
students through an awarding ceremony.

Program Preparation:
A day before the program, the following teachers were assigned
accordingly:

Program Results:
Before the set time, some parents have arrived earlier as
compared to expected time. To facilitate the unexpected situation, the
grade advisers were instructed to check the attendance of the parents
accordingly. They were also guided on their presupposed seats intended
for them. Some advisers have also found some time to talk to concerned
parents with regards on the problems being manifested by the students
at the school.

The program, facilitated by Mr. Norman Battulayan and Mrs.


Josphine Baniaga, started at around 8:00 P.M with a prayer and
nationalistic song led by Mrs. Marikarl Arellano, MAPEH teacher. An
opening remarks were made by Mr. Franciso S. Tattao, principal,
highlighting the rationale and the importance of the said activity to
both students and parents. He also mention some updates on the status
of the school improvement and management. Mrs. Anabelle Ugalino, ADAS,
also presented the financial report and status of the school in
support of the update made.
After which, selected students from each grade were asked and
made to present in the stage:
Grade level: Description of the Presentation:
12 Dance - Musical Number intended to present the
concepts and principles of Business subjects in real
life situation.
11 Speech Choir intended to present their accumulated
skills in their Communication skills subjects
10 Drama intended to present their skills and
appreciation of Philippine literature
9 Musical numbers intended to present their basic skills
8 accumulated in their MAPEH subjects.
7 Tula (Poem Interpretation) showcasing their skills
accumulated from Filipino Subject
7 – STE Musical Dance number intended to present their
understanding on basic concepts in Mathematics
focusing on Operations of Integers.

After the lengthy presentations, recognition program of academic


and co – curricular achievements of the students were presented:

12 2 students were awarded with high honors
4 students were awarded with honors
Perfect attendance were awarded to deserving students
Others were awarded for the Science – Math Fair
11 Only 2 students from STEM strand were awarded with
honors, none from the ABM Strand
Perfect attendance were awarded to deserving students
10 __ students were awarded with honors
Others were awarded for the Science – Math Fair
Perfect attendance were awarded to deserving students

After the recognition program, the parents were advised to


proceed to the classroom of their sons/daughters to get the report
cards of the students and settle some problems or concerns (if there
are any) with the class advisers.
The program ended at around 12:00 P.M.
Recommendations:
Based on the results, it is recommended that continuous activity
similar to these would be conducted as it showcase the curriculum
experiences of the students.
It is however pointed out that certain presentations must be made
shorter in order to lessen the time spent for the whole duration of
activity since it has been observe that some parents were leaving
ahead of time due to “work” reasons.

ACTIVITY TITLE: RECOGNITION AND CARD GIVING DAY OF STA. MARCELA
NATIONAL HIGH SCHOOL FOR THE SECOND QUARTER OF
THE SCHOOL YEAR 2018 – 2019

SUMMARY OF ACCOMPLISHMENT:

Background and Rationale:


Previously, it was planned that a second culminating activity
will also be done due to its perceived good results, however, due to
many reasons, the said activity was not realize, instead a recognition
and card giving day were conceive as alternative activity in order to
meet the needs of the learners to be extrinsically motivated to their
learnings.
Further, this activity was conceive as an avenue to advisers and
parents to have an ample time to talk and plan for the academic
remediation of those students who have problems in school.
Hence, SMNHS, through the initiative of the Principal, conducted
a simple recognition program and card giving day to be held separately
in each classroom.

Program Objectives:
Through the conceived activity, the following objectives were
then determined:
1. To recognize academic and extra – curricular achievement of the
students through a simple awarding ceremony; and
2. To map out possible remediation of students who have problems in
their studies in school.

Program Preparation:
A day before the activity, the class advisers were advised to
prepare the necessary documents and materials such as Cards, Reports
and other files as deemed necessary.
Rooms were also cleaned and prepared for conduciveness. Further
students were given letters informing their parent for the said
activity.

Program Results (on the case of Mrs. Jona Lopit):


As expected, some parents started to arrive earlier as expected.
They were immediately instructed to proceed inside the classroom while
waiting for the other parents to arrive. The program begins 15 minutes
after the set time to accommodate those parents who were late.
The activity started with a simple prayer. After which, checking
of attendance were made in which parents were made to sign on the
prepared attendance sheets. Introduction and pleasantries were also
made in order to create positive climate among and between the parents
and the adviser.
The first part of the activity were the reports and
accomplishments as of the second quarter of the year which includes
the purchase and installation of Television which will be used mainly
for instruction intended to digitize classrooms for the 21st century
learners.
After, recognition and card giving day were made accordingly.

Recommendations:
Based on the results, it is recommended that activities similar
to these would be conducted as it provides continuous monitoring of
the academic achievement of the learners. Further, it allows the
teacher and parents to determine possible avenues in which they could
provide a more conducive classroom to learners.
