TRAINING OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement, made and entered in Lucena City, this day of August
2017 by and between:

Municipality of Pagbilao
Pagbilao, Quezon

And

The CALAYAN EDUCATIONAL FOUNDATION, INC., (CEFI) an educational

institution duly recognized by the Commission on Higher Education with principal
office at Marhalika Highway, Red V Lucena City, represented by its President, Atty.
RONALDO D.V. CALAYAN hereinafter referred to as the “Institution”

WITNESSETH

1. That the establishment agrees and allows the 480 Hours On the Job Training of
Information System Students of the Institution subject to the policies, procedures
and guidelines governing training of students.

2. That the establishment shall be responsible for the implementation and

enforcement of pertinent laws and regulations governing training. Likewise, the
Institution shall strictly comply with any provisions of law pertinent to training
and any violation thereof shall be legitimate ground for cancellation of the
contract.

3. That this contract is effective for one (1) year and shall take effect of August 7,
2017 to December 15, 2017 subject to annual renewal and review by the
Department Chair of Information System of the Institution, who may
recommend its termination if such prove disadvantageous to proper functioning of
the Institution and establishment or detrimental to the safety and welfare of the
students and/or personnel of both parties.

4. That the admissions of students/trainees in terms of qualifications shall be based

on the screening and/or exams to be given by the Institution.

5. That establishment shall allow the Information System student-trainee to

observe and evaluate present system, assist in the maintenance and upgrading
of system/ programs and other related works in their establishment.

6. That the training hours of each student will be five (5) days in a week

7. That the Information System student-trainee shall not be allowed to work on

overtime or during holidays, Saturdays and Sundays.

8. That the establishment thru the undersigned head/student trainer takes

responsibility of training the Information System Students while he/she is on
duty or performing official functions.

9. That the establishment thru its department head/student trainer shall comply
with school evaluation requirement for student-trainees.

10. That any misconduct or other form of misbehavior on the part of the student
trainee shall be ground for immediate termination of the contract upon sufficient
investigation. The establishment also reserves the right to discipline erring
student trainee and to impose sanction, including expulsion when necessary.
