JOB DESCRIPTION

POST: Clinical Fellow in Critical Care, Emergency Care, Trauma and


Resuscitation – up until 3 August 2021

LOCATION: Critical Care, Unscheduled & Acute Services

REPORTS TO: Specialty Improvement Lead, Major Trauma service

RESPONSIBLE TO: Clinical Director, Major Trauma Service

The Belfast Health and Social Care Trust

The Belfast Trust delivers integrated health and social care to approximately 340,000
citizens in the greater Belfast area and provides the majority of regional specialist services
for the population of Northern Ireland (1.8 million people). With an annual budget of almost
£1.3bn and a workforce of over 20,000, we are also a Trust with teaching hospitals and
have strong links with our local university partners providing high quality training for medical
and other health care professionals across all disciplines.

The Royal Hospitals is the largest hospital complex in Northern Ireland, comprising the
Royal Victoria Hospital, the Royal Jubilee Maternity Hospital (RJMH), the Royal Belfast
Hospital for Sick Children (RBHSC) and the Dental Hospital. There are approximately 900
beds on the site and a high proportion of the regional specialties for the province are
concentrated in the complex. These specialties include vascular surgery, cardiothoracic
surgery, trauma, cardiology, neurosurgery, neurology, ophthalmology, plastic surgery and
burns, otorhinolaryngology, paediatrics and neonatology.

The Emergency Department is the largest in Northern Ireland. Acute medicine, respiratory
medicine, stroke medicine, and general surgery are also located on this site.

The Royal Jubilee Maternity Hospital is the largest Maternity Hospital in Northern Ireland with
in excess of 5000 deliveries per annum. It is the tertiary referral centre for high risk obstetrics.

The Royal Hospitals play a major role in clinical education, training and research, with most
academic departments linked to the Queen’s University of Belfast Medical School on the
Royal Hospital’s complex – medicine, surgery, ophthalmology, child health, obstetrics and
gynaecology, and pathology. The Royal Victoria Hospital is a Level 1 Trauma Centre and
supports the Northern Ireland Trauma Network and Northern Ireland Air-Ambulance Service.

The Regional Intensive Care Unit (RICU) is located in the Royal Victoria Hospital.

RICU is the largest critical care unit in Northern Ireland. It comprises 19 Level 3 and 8 Level
2 beds. There are approximately 1200 adult admissions per year. RICU is the regional centre
for trauma, thoracic surgery, neurosciences, burns, infectious diseases and hepatology.
Approximately 88% of all admissions are unplanned surgical, trauma, or medical admissions.
The Northern Ireland Critical Care Transfer Service (NICCATS) is coordinated from RICU.

The unit contributes to the ICNARC Case Mix Programme. Standardised mortality rates are
among the best in the United Kingdom for similar profile critical care units.

The Critical Care Department has 19 consultants (see below), two of whom have Joint
Appointments with the Queen’s University of Belfast.

There are two associate specialists working in RICU. Currently there are 15 doctors in
training, from the Foundation programme, core medical training, respiratory medicine,
anaesthesia and critical care. The posts in RICU are recognised for all levels of training in
critical care.

There is an active program of internationally recognised research. Acute lung injury is one of
the main research areas.

The training of junior staff is supervised by the Education Committee of the Anaesthetics and
Critical Care Department. RICU has a tutor from the Faculty of Intensive Care Medicine and
two departmental educational supervisors, who oversee the training of all doctors in training
while in RICU.

There is a strong ethos of Quality Improvement in RICU and all staff actively participates in
Q.I. activities. There are regular departmental morbidity and mortality, audit, and journal club
meetings, which all staff are expected to attend. There is weekly scheduled teaching for
trainees and regular seminars co-ordinated by the Education Committee of the Directorate.
Support is provided for trainees preparing for professional examinations.

The Faculty of Medicine at the Queen's University of Belfast trains in excess of 250 medical
students each year and a high proportion of the training takes place in the BHSCT hospitals.
Medical students are on regular placements in RICU. The main Medical Library in the
province is situated in the Institute of Clinical Science on the Royal Hospitals site.

Belfast City Hospital is one of the major teaching hospitals in the BHSCT, located adjacent
to the Queen’s University of Belfast. It provides the following surgical specialties: colorectal,
upper gastrointestinal, renal transplantation, breast (including reconstruction surgery),
gynaecology, and urology. It contains the cancer centre for NI, haematology, nephrology,
cardiology and respiratory medicine. For many years the Belfast City Hospital has been
foremost in the province in the care of patients with chronic pain. It has a 9 bedded critical
care unit (5 Level 3 and 4 Level 2), which is staffed by 8 consultants.

The Mater Hospital is a tertiary referral centre for pancreatic and hepatobiliary surgery. It
has a 6 bedded critical care with (2 Level 3 and 4 Level 2). It has a 24 hour Emergency
Department. It is a long established general hospital with teaching status affiliated to the
Queen’s University of Belfast. A new state of the art ward block, the McAuley Building, was
opened in January 2002. The Dempsey Building, which was opened in 1991 houses the
Emergency Department, the Operating Theatres, the Intensive Care/High Dependency Unit,
the Outpatient Department, the X-Ray Department and the Maternity Unit and the main
Psychiatric Unit occupies a separate building next to the Dempsey Building. There is also a
Psychiatric Day Hospital on a separate site, approximately one mile from the main hospital.
The Major Trauma Service was established in January 2019. It co-ordinates the care of
polytraumatised patients admitted to the the regional Major Trauma Centre. There are
approximately 300 admissions per year.

Musgrave Park Hospital is the Regional Orthopaedic Unit for Northern Ireland. The
Musgrave Park Regional Orthopaedic Service is the largest in the British Isles with 48
consultant orthopaedic surgeons and staff. On site is the Queen’s University of Belfast’s
Department of Orthopaedic Surgery which is the largest academic unit in Orthopaedics in the
British Isles with an international reputation and an extensive research output.

Main Duties / Responsibilities

Specialised Fellowship Activity

Resuscitation and emergency management are major elements in the care of the
polytraumatised patient. This fellowship will allow the post holder to develop a wide range
of skills in all aspects of resuscitation of the critically ill. The post will be based in the Major
Trauma Ward with sessions in the Regional Intensive Care Unit. As part of the critical care
team, the Fellow will be involved in the management of patients in the unit and on the ward.
The fellow will also attend resuscitation, critical care and trauma referrals in the Emergency
Department as part of TEAM RESUS. They will also help provide an outreach service for
trauma patients on the surgical wards.

The Fellowship will allow the post holder to develop many of the practical skills required to
manage critically ill patients. They will acquire the skills required for complex venous
access. There will be the opportunity to be involved in the transfer of critically ill patients.

This post would be suitable for doctors with a background in Emergency Medicine,
Anaesthesia, or the acute medical and surgical specialties who wish to develop their
knowledge, skills and competence in the management of the polytraumatised patient.

This post would also be suitable for post CCT trainees who wish to develop their knowledge
and skills in the management of the critically ill patient.

The post holder will work under the direction of the fellowship supervisor Dr P. O’Connor,
and Mr D Kealey, the Clinical Director for Major Trauma. It is expected the post holder will:

1) Provide critical care services for patients in the Regional Intensive Care Unit.

2) Undertake the Fellowship roles and responsibilities highlighted above.


3) Participate in a resident on-call rota in RICU.

4) Work with professional colleagues within the critical care team, other professionals
involved in the provision of patient care, and with the management of the Trust

5) Carry out administration work arising from the clinical commitment.

6) Keep a logbook of airway interventions and vascular access procedures performed.

7) Participate actively in undergraduate and postgraduate teaching of the department.

8) Participate in departmental quality improvement activities.

9) Undertake regular appraisal with a Clinical Supervisor.

10) Meet objectives agreed with the Fellowship supervisors.

11) Ensure that mandatory training requirements (eg Right Patient Right Blood) are
satisfied.

(which should not unreasonably be withheld) with the post holder.

Governance

The successful candidate will be expected to work within the Belfast Trust HSC Trust
governance framework. The Trust is committed to conducting a process of yearly appraisal
of medical staff and will be used in future to support CME revalidation. An initial job plan
review will be held with the Clinical Director within 3 months of the candidate taking up post.

Quality Improvement, Multiprofessional Audit and Continuing Medical Education

The post holder will be required to take part in the department’s quality improvement and
audit programmes. They will also be required to undertake such continuing educational
activities as are deemed necessary by the Faculty of Intensive Care Medicine.

Teaching and Research

The successful candidate will be expected to participate in the teaching of undergraduate


medical students and the training of junior medical staff and other professional staff.

Administrative Facilities

The post holder will have access to computer facilities with e-mail/internet. He/she will
share a Secretary with other members of the Team.

Superannuation

Membership of the Health Services Superannuation Scheme is voluntary. There is


reciprocity between the Northern Ireland scheme and that in mainland Britain.

Medical Fitness

Appointment to this post is subject to a satisfactory pre-employment health assessment


with the Trust’s Occupational Health service.

Study Leave/ CPD

Study leave must be applied for in advance and only courses that are deemed appropriate
will be funded, as per Trust policy. It is expected that CPD activities will be carried out in the
hospital where appropriate. A library is located on the Hospital site for this purpose.

Registration

Required to maintain full and specialist registration (and hold a current licence to practise)
with the General Medical Council

Canvassing

Canvassing will disqualify. Individual applicants nor other persons should canvass support
for any application in respect of Consultant posts.

Changes

who are responsible for cleaning services, all Trust staff have a responsibility to ensure a
clean, comfortable, safe environment for patients, clients, residents, visitors, staff and

Infection Prevention and Control

The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all
staff have a part to play in making this happen. Staff must comply with all policies in relation
to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection
Prevention and Control Precautions must be used at all times to ensure the safety of patients
and staff. This includes:-
 Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO
‘5 moments’);
 Using the correct ‘7 step’ hand hygiene technique;
 Being ‘bare below the elbows’ when in a clinical environment;
 Following Trust policies and the Regional Infection Control Manual (found on intranet);
 Wearing the correct Personal Protective Equipment (PPE);
 Ensuring correct handling and disposal of waste (including sharps) and laundry;
 Ensuring all medical devices (equipment) are decontaminated appropriately i.e., cleaned,
disinfected and/or sterilised;
 Ensuring compliance with High Impact Interventions.

JOB SPECIFICATION

POST: Clinical Fellow in Critical Care, Emergency Care, Trauma and Resuscitation

LOCATION: Critical Care, Unscheduled & Acute Services

ESSENTIAL CRITERIA

 Full Registration with the General Medical Council and a Licence to Practice (must have
both at time of appointment).

 Candidates must have completed Foundation Programme training (or its equivalent
gained on a part time or flexible basis) or
Shall have equivalent experience and competencies.

 All applicants to have demonstrable skills in written and spoken English that are
adequate to enable effective communication about medical topics with patients and
colleagues which could be demonstrated by one of the following:
That applicants have undertaken undergraduate medical training in English;
OR
Have the following scores in the academic International English Language Testing
System (IELTS) – Overall 7.5, Speaking 7, Listening 7, Reading 7, Writing 7
OR
Have taken the medical version of the Occupational English Test (OET), achieving at
least a grade B in each testing area (Speaking, Listening, Reading and Writing) in the
same test and have obtained them in the most recent sitting of the test

However, if applicants believe that they have adequate communication skills but do not
fit into one of the examples they need to provide evidence.

 Ability to work collaboratively within a multidisciplinary team.

 Commitment to self-development through continuing medical education

DESIRABLE CRITERIA

 Success in postgraduate examinations relevant to either anaesthesia, Emergency


Medicine or an acute medical or surgical specialty.

 Previous experience in managing acutely ill patients.
