GERONIMO CORONEL DANTES

I, GERONIMO CORONEL DANTES, of legal age, Filipino, single,

and with postal address at 1185 Lambakin, Marilao, Bulacan, after
having been duly sworn to an oath hereby state:

PRELIMINARY STATEMENT

I am being questioned by Atty. Cristopher Jon Reyes with office

address at No. 27 Little Baguio St., Marulas, Valenzuela City. The
questioning is being held at the same office. I am answering his
questions fully conscious that I do so under oath and may face criminal
liability for false testimony.

This Judicial Affidavit is being offered to prove the following:

1. That the Petitioner is the registered owner of a parcel of land at

situated at Marilao, Bulacan and covered by Transfer Certificate of
Title No. 040-2018008537;

2. The facts and circumstances surrounding the loss of Transfer

Certificate of Title No. 040-2018008537;

3. All the allegations in the Petition including all annexes appended

thereto;
 4. All other relevant matters, facts and circumstances relevant and
material to this case.

QUESTIONS AND ANSWERS

QUESTION NO. 1: Please state your name and personal

circumstances for the record.
ANSWER: I am Geronimo Coronel Dantes, of legal age,
Filipino, single, and with postal address at 1185
Lambakin, Marilao. Bulacan.

QUESTION NO. 2: Are you the same Geronimo Coronel Dantes who is
the Petitioner in this case?
ANSWER: Yes.

QUESTION NO. 3: What is the purpose of your Petition in this case?

ANSWER: For the issuance of a new owner’s duplicate copy
of Transfer Certificate of Title No. 040-
2018008537;

QUESTION NO.4: Why are you seeking the issuance of a new

Transfer Certificate of Title No. 040-2018008537?
ANSWER: I am the registered owner of a parcel of land
covered by Transfer Certificate of Title No. 040-
2018008537.

Being the registered owner, I have the

corresponding owners duplicate copy of the
aforementioned Transfer Certificate of Title.

I have kept the copy of the transfer certificate of

title inside a brown envelope for safe keeping. The
same brown envelope was kept inside my
residence but one day during a routine clean-up of
my residence, I noticed that I was unable to locate
the said brown envelope containing my owners
duplicate copy.

After a diligent search, I was still unable to locate

the said brown envelope containing my owners
duplicate copy of the Transfer Certificate of Title.

QUESTION NO. 5: I am showing to you a Certified True Copy of Title

No. 040-2018008537. Is this the certified true
copy of Transfer Certificate of Title No. 040-
2018008537 of which you are the registered
 owner?
ANSWER: Yes.

QUESTION NO. 6: I am marking this Transfer Certificate of Title No.

040-2018008537as Exhibit “E“ and attaching the
same to your Judicial Affidavit. Do you confirm my
action?
ANSWER: Yes

QUESTION NO. 7: What did you do upon discovering that your

owner’s copy of Transfer Certificate of Title No.
040-2018008537was missing?
ANSWER: I have executed an Affidavit of Loss affirming that
my copy of the Transfer Certificate of Titles was
missing.

QUESTION NO. 8: I am showing to you an Affidavit of Loss executed

by a certain Geronimo Coronel Dantes. Is this the
same affidavit of loss that you have executed?
ANSWER: Yes

QUESTION NO. 9: I am marking this Affidavit of Loss as Exhibit “F“

and attaching the same to your Judicial Affidavit.
Do you confirm my action?
ANSWER: Yes

QUESTION NO. 10: What other steps did you do?

ANSWER: I have caused the annotation of the loss of my
owner’s copy of my Transfer Certificate of Title.

QUESTION NO. 11: I am showing to you an annotation located at page

3 of the Certified True Copy of Transfer Certificate
of Title No. 040-2018008537. Is this the
annotation that you have made?
ANSWER: Yes

QUESTION NO. 12: I am marking the annotation for the loss of the
owner’s duplicate copy for the Certified True Copy
of Transfer Certificate of Title No. 040-
2018008537 as Exhibit “E-1“.Do you confirm my
action?
ANSWER: Yes

QUESTION NO. 13: What other proofs can you show that you are
indeed the registered owner of the parcel of land
covered by Transfer Certificate of Title No. 040-
2018008537?
ANSWER: I have been paying the corresponding real
property taxes for the aforementioned property as
well as the improvements situated therein.

QUESTION NO. 14: I am showing to you Tax Declaration TD/ARP. No.

2018-12006-19028 Is this the tax declaration for
Transfer Certificate of Title No. 040-2018008537?
ANSWER: Yes.

QUESTION NO. 15: I am marking Tax Declaration TD/ARP. No. 2018-

12006-19028 as Exhibit “G“ and attaching the
same to your Judicial Affidavit. Do you confirm my
action?
ASNWER: Yes.

QUESTION NO. 16: I am showing to you Tax Declaration TD/ARP. No.

2018-12006-19029. Is this the tax declaration for
the improvement situated in the parcel of land
covered by Transfer Certificate of Title No. 040-
