We are respectfully submitting this letter in reply to the evaluation findings with
regards to our request for the renewal of our POEA license.

Attached herewith are the amended and additional documents.

1. Applicant Information Sheet for Renewal of License (FM-POEA-03IR-02B) signed

by the Head of Office.

Comments:
 SINCE THE MAIN OFFICE OF THE AGENCY IS IN BENGUET, INCLUDE ONLY THE
NAMES OF STAFF IN THE MAIN OFFICE (EXCLUDE THOSE IN THE BRANCH
OFFICES):
- RODA SIMBUL LIMPIN IS ASSIGNED IN MANILA BRANCH BASED ON THE
POEA RECORDS;
- JENNILYN CHLOE NAVAL DELA CERNA IS BRANCH MANAGER IN MANILA
BRANCH;
- GERALDINE CORCUERA BARASI IS BRANCH MANAGER IN PASIG BRANCH;

 The above-mentioned names were removed from the list of officers and
staffs in the main office. Attached is the amended applicant information
sheet.

 CLARIFY THE STATUS OF THE FOLLOWING:

- ANNALIZA VALDERAMA PADUL who is active in the database but is not
included in the applicant information sheet;

 Resignation letter of Annaliza Valderama Padul and notice of her resignation

was submitted and received by your office on July 2, 2020. Copy of the
notice with stamp of acknowledgement was emailed to us on July 7, 2020.

 - THE ACKNOWLEDGEMENT OF VITALIANO SANTOS CORCUERA JR. AS
PRESIDENT WAS ISSUED 14 SEPTEMBER 2014. HIS POSITION DURING THE
2016 RENEWAL ARE AS DIRECTOR AND CHAIRMAN BUT THE APPLICANT
INFORMATION SHEET DECLARES HIM AS THE PRESIDENT OF THE AGENCY.

 I, Vitaliano Santos Corcuera Jr., was elected and appointed as the President
(and Chairman of the Board of Directors) of Faasi International Corporation
on February 2014. Attached is the copy of notice of publication.
My election as the President of Faasi was acknowledged by the Licensing
Branch, POEA on September 14, 2014. Attached is the copy of POEA
acknowledgment.
During the 2016 renewal of our license, my position encoded in the applicant
information sheet under the Board of Directors is “Chairman” and under the
position of officers is also “Chairman”. It was my understanding and
believing in good faith that “Chairman and President” have the same and
one meaning and therefore can be used as one or interchangeably. It was an
honest mistake that I failed to indicate my position correctly as “President”
under the position of officers. Also, when we filed and submitted our
application for renewal, the “Chairman” specified as my position was not
given attention. Attached is a copy of the applicant information sheet filed on
2016.
Since February 2014 up to present, no election or appointment of new
officers was made and I am still the incumbent President (and Chairman of
the Board of Directors) of Faasi International Corporation.

- JENICA ALCALA GIGANTE, MESCHELLE PARDITO DELA CERNA, JOSIE NAVAL

LOMAAD, ADRIN CRUZ NAVAL AND RYAN MACARAEG, who are active and
assigned in the main branch and were included in the original submission
of the applicant information sheet but are no longer included in the recent
submission.

 The above-mentioned staffs, their position and acknowledgment date were

included in the amended Applicant Information Sheet for Renewal of License
as staffs of main office.

 MENCI FERNANDEZ TOLERO JR. is a shareholder but not a director based on

the GIS, hence remove his name in the Applicant Information Sheet for
Renewal of License as Director.

Applicant Information Sheet for Renewal of License.

3. Escrow Agreement

Comments:
 THE ESCROW AGREEMENT SHOULD BE SIGNED BY THE PRESIDENT OF THE
AGENCY
 SUBMIT AUTHORITY TO EXAMINE BANK CERTIFICATE

 Attached is the Escrow Agreement duly signed by Ms. Geraldine C. Barasi,

the designated authorized signatory on the Escrow Agreement. As per PNB
Trust Banking Group, on the Board Resolution and Secretary Certificate, Ms.
Geraldine C. Barasi and Ms. Evelyn F. Tolero, any one/any two are the duly
authorized singnatories of the corporation. Attached are copies of Board
Resolution and Secretary Certificate submitted to PNB Trust Banking Group.
 The Authority to Examine Bank Certificate.

5. All Directors, Officers and Staff

Comments:
 SUBMIT THE VALID AIRB CLEARANCE OF VILMA SANTOS CORCUERA

 We regret to inform your good office that as of now we cannot submit the
valid AIRB Clearance of Vilma Santos Corcuera.

We cannot request for the issuance of AIRB Clearance of Vilma because one
of the requirements is her valid NBI Clearance. Vilma is still in Japan and she
will be arriving this October 2020. As soon as she will be here in the
Philippines, she will have her personal appearance at the NBI since she
already had her confirmed appointment on November 6, 2020. Once her NBI
be issued and released, we will immediately request for her AIRB Clearance.

In this regard, we respectfully submitted already an Affidavit of Undertaking

(executed and signed by the President of Faasi International Corporation)
that said valid AIRB Clearance be simultaneously submitted with her NBI
clearance within sixty (60) days from the date of the execution of the
affidavit of undertaking per our Appeal letter dated September 1, 2020 and
your email reply dated September 4, 2020. Attached are copies of Appeal
letter, reply email dated September 4, 2020 (re: submission of AIRB Clearance
of Vilma S. Corcuera)

  ON THE APPOINTMENT OF THE INCOMING PRESIDENT, VITALIANO SANTOS
CORCUERA, JR. SUBMIT REQUIREMENTS FOR THE NEW PRESIDENT / NEW
DIRECTOR.

 I, Vitaliano Santos Corcuera Jr., is the incumbent President of Faasi

International Corporation since February 2014 up to this date. There was no
election or appointment conducted for a new president or director.

7. A duly notarized Affidavit of Undertaking (AU) per Section 4 (G) of 2016 Rules
(Landbased and Seabased)

Comments:

 THE AU PER SECTION 4(G) SHOULD BE SIGNED BY THE BOARD OF DIRECTORS

ONLY

 Attached is the Affidavit of Undertaking per Section 4(G) signed by the Board
of Directors only.

8. Certificate of Attendance to the Comprehensive Agency Education Program

(CAEP) of all its officers and staff.

Comments:

 REVISE THE AFFIDAVIT OF UNDERTAKING WHICH SHOULD BE SIGNED BY THE

PRESIDENT OF THE AGENCY AND INDICATE THE SCHEDULE OF CAEP SEMINAR
AND ATTACH A COPY OF THE OFFICIAL RECEIPT OF PAYMENT, MOREOVER,
INDICATE ONLY THE NAME OF DIRECTORS, OFFICERS AND STAFF IN THE MAIN
OFFICE.

 Attached is the revised Affidavit of Undertaking signed by the President of

Faasi International Corporation, indicating the name of directors, officers
and staff in the main office.

We cannot indicate the schedule of CAEP SEMINAR and attach a copy of

official receipt of payment for the reasons that:
- last September 19, 2020, we sent an email at caep@poea.gov.ph
requesting for a CAEP schedule.
- On September 21, 2020, we received a telephone call from Ms. Angie of
ERB, POEA – informing us that she cannot give schedule for CAEP because
under the POEA system, our license was cancelled. We inform Ms. Angie

 that we are on the process of renewal and she said that we can request
for schedule (since CAEP is conducted daily) once our license is renewed.

10. Valid Certificate of Compliance with general labor standards and valid certificate
of compliance with occupational safety and health standards.

Comments:
 SUBMIT ORIGINAL COPY – DOLE CERTIFICATE OF NO PENDING CASE ISSUED BY
DOLE CORDILLERA ADMINISTRATIVE REGION (CAR)

 The original copy of the DOLE Certificate of No Pending Case issued by the
DOLE-CAR was attached/enclosed with the other folder “Office Copy” that
we submitted to your office. Kindly check.
