1 month PTA, patient developed fever with temperature of 380 C with chills. No other
accompanying symptoms such as vomiting, diarrhea, cough and colds, no headache. Consultation was
done, CBC with platelet count required o assess dengue, negative results, prescribed with Paracetamol,
Mefenamic acid and Ciprofloxacin. 3 weeks PTA, patient noted rashes on face, trunk UE, LE with fever. 2
weeks PTA, patient was confined at Sta. Teresita Hospital, due to weakness and decreased appetite,
assessed with measles some meds prescribed. On discharge, patient still had fever and Widal͛s test
done. Showing positive for Salmonella typhi O,H, patyphi A & B. patient was confirmed to have fever
with chills. He was prescribed with Chloramphenicol 500mg 1 tab q 6o and amoxicillin 500mg q 80.
Patient was compliant but due to persistence of symtoms, patient seeks consultation and he was
admitted in San Lazaro Hospital. His initial Vital signs were T = 36.90C, P = 84 bpm, BP = 110/80, and R =
20 cpm.

6 years ago, the patient was hospitalized in Philippine General Hospital due to pain that radiates
to flank area. It was diagnosed that he has renal calculi. He had undergone surgery to remove the calculi
in his kidneys.

1 year ago, the patient was again hospitalized due to a bump in his beside his nose. The bump
started as a pimple-like structure then days passed, it persisted and it grew, it has also pus. He did not
take any medications and he seeks for consultation. The doctor advised him to undergo surgery to
remove the bump because according to the doctor, it might affect his whole face. He had undergone a
surgery to remove the bump.

The patient has also history of smoking. He started smoking when he was 20 y/o and he stopped
it in 2009 (from 1971 to 2009, 38 years). He usually consumes half pack of cigarettes per day. (Pack
years = 19)

He does not have any allergies to any foods or medications.


 

According to the patient, there are no known familial diseases in their family. They do not have
any diseases such as Hypertension, DM or TB.
