The following forms are for volunteers to fill in, one for each group in your Visit Three form group. The information in
the left-hand column of the tables is the exact information that they submitted during Visit Two – it may have
changed by this visit, so you need to amend it accordingly. Please confirm or alter the details that we have already
collected, so that we keep up with how students’ mini-projects have evolved.

Name of mini-project
 If the group has no name that’s fine as we won’t use this information for anything.

Full names of group members


 These are the names that they filled out in their Visit Two forms.
 If someone has left or joined the group, alter the section as appropriate.
 The names that will appear on certificates will be the names on the register (so it might be the non-nickname
form of the name that the student has written).

Brief description of mini-project


 We will use this information to remember what all the mini-projects were when deciding on the top 10
groups for prize-giving.
 If the mini-project has changed a lot, alter the section as appropriate (in 1-2 sentences).

Output forms
 The electronic output forms (e.g. video, Powerpoint) can easily be checked by you.
 Other output forms (e.g. event, bake sale) has to be verified in another way e.g. with photographic evidence,
or by you actually seeing the product (e.g. if it’s a poster designed by hand).
 Alter as appropriate to document what the group actually ended up producing.

Additional information
 Anything else that you’d want to remember later when it comes to judging.
 The preferred name of a student who doesn’t want their register name on their certificate (e.g. if it conflicts
with their gender identity).
 Special circumstances or special observations you’ve made about the group.
 A plan of action in the case that the group cannot submit their mini-project today, or need to submit it in
another way, or cannot submit it.

Many sports are seen to be suitable to certain types of
people. Our project will aim to highlight those who are
LGBT, go against some stereotypes in sport and highlight
how some sportspeople and sports go against the
