“E” The Marriage Contract of Petitioners; Offered to prove as part of
the testimony of the
petitioners and to prove
that they were married on
September 5, 1992 at Bato
Catholic Church, Toledo
City;

Also offered to prove that

in the said marriage
contract, the name of the
entry for husband was
erroneously entered into as
Paulino D. Talon, when in
truth and in fact, the name
of the entry should have
been Mario D. Talon;

During the marriage

Ceremony, petitioners
entered the name of
husband as Paulino

 because that is the name
that appears in his
Baptismal Records, it was
only later when they were
able to secure the Birth
Certificate of Petitioner
Mario D. Talon that his
actual name is Mario D.
Talon;

“F” Petition in Special Proceedings No. 1163-T Offered as part of the

Driver’s testimony of the petitioner
Mario Talon and to prove
that he, together with his
spouse Elenita Talon filed
a petition before the
Regional Trial Court of
Toledo City to correct the
erroneous entry in their
marriage contract;

“G” NSO-issued Birth Certificate of Petitioner Offered to prove that in

Mario Talon truth and in fact, the name
of petitioner is Mario D.
Talon and not Paulino D.
Talon;

“H” NSO-issued Birth Certificate of Ian L. Talon Offered to prove that in the
NSO-issued Birth
Certificate of Ian L. Talon,
the name of the father
erroneously appears as
Paulino D. Talon;

“I” NSO-issued Birth Certificate of Christian L. Offered to prove that in the

Talon NSO-issued Birth
Certificate of Christian L.
Talon, the name of the
father erroneously appears
as Paulino D. Talon;

“J” Driver’s License of Petitioner Mario D. Offered to prove that

Talon petitioner Mario D. Talon
was issued a Driver’s
License by the Land
Transportation Office
under the name Mario D.
Talon;

 “K” Voter’s Identification Card of Petitioner Offered to prove that
Mario D. Talon petitioner Mario D. Talon
was issued a Voter’s
Identification Card by the
Commission on Election;

Exhibits “E” is already attached to the record of the case, while exhibit
“G” to “K” are hereto attached.

WHEREFORE, in view of the foregoing, petitioners pray to this Honorable

Court that the foregoing exhibits be dully admitted;
