COMMUNITY ASSOCIATION SERVICE CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This contract for Manpower services for manual labor is entered into by and between

PANTOW KOL-LELAN ASSOCIATION, a duly registered non-stock, non-profit community

organization existing under the laws of the Republic of the Philippines, located at Purok 7, Brgy.
Tamayong, Calinan District, Davao City, Philippines herein referred to as LABOR PROVIDER
represented therein by its president MR. ERNESTO SICAO;
AND
The Philippine Eagle Foundation, INC. , non-stock, non-profit organization, existing under the
laws of the Republic of the Philippines, with principal office at the Philippine Eagle Center,
Malagos, Baguio District, Davao City herein referred to as the FOUNDATION represented
therein by its Community Livelihood Development Officer MR. ALPHEUS SHEM A. ROJAS;
WITNESSETH
1. That the foundation has requested the services of the PANTOW KOL-LELAN
ASSOCIATION for the Livelihood component of “FOREST PROTECTION cum
SUSTAINABLE LIVELIHOOD PROJECT “ from June 26, 2017 to OCTOBER 26, 2017.
2. That the LABOR PROVIDER hereby binds to render services to the FOUNDATION
starting July and will do the following:
a) The LABOR PROVIDER will report to the community livelihood development
officer all services rendered.
b) The LABOR PROVIDER will be responsible for ensuring adequate manpower for
manual hauling and planting of seedlings in the farm plots.
c) Assist the community livelihood development officer in the follow-through
facilitation of farm plot maintenance.
3. That for the consideration of the above deliverable output by the LABOR PROVIDER the
FOUNDATION shall pay in cash the amount of two pesos per seedling hauled and that
after all of the sixty thousand seedlings are hauled and planted, a total of One Hundred
Twenty Thousand Pesos (Php 120,000.00) as payment for services rendered;
IN WITNESS HEREOF, the parties hereto set their hands this day of 2017 at
Malagos, Baguio District, Davao City Philippines.
