Scene 1

Neham Usman,wears a clean, short sleeved cotton scrub suit

before entering the operating department. The scrub suit
covers her other clothing such as underwear, and trouser
legs, not touching the floor as this may transport bacteria
from one place to another the scrub nurse.

Scrub nurse then explains the importance of scrub

practitioners training either as pre registration operating
department practitioners (ODPs) or as post registration
nurses. Emphasizing scrub practitioners must have a thorough
knowledge of operating room procedures, including the
instruments needed for specific surgery, and must be able to
stay calm and clear headed even under pressure.
Perioperative practitioners provide patient care before,
during and after surgical procedures.The role of scrub
practitioners is to support and assist surgeons during the
surgical procedure, to ensure the best, safest and most
effective care for the patient. To do this they must have
knowledge of anatomy and physiology, surgery, and the
instruments and equipment needed for the procedure. An
experienced scrub practitioner supports the surgeon in
undertaking a smooth and efficient procedure, and can
anticipate what is needed to prepare efficiently and prevent
waste.

Christine Salimbagat, wears a clean, short sleeved cotton

Circulating nurse Christine Salimbagat then explains the

importance of her responsibilities of setting up the
operating room correctly. Where she also checks all
equipment needed during the procedure to verify that it is
functioning normally. When the patient arrives in the OR,
the circulating practitioner usually verifies the patient’s
identity and necessary consent forms and then reviews the
site and nature of the procedure with the surgeon.
Circulating practitioners also work to promote the
cleanliness and sterility of the operating room and tell
operating room staff of anything that may cause
contamination.

Scene 2
--------------------BEFORE THE SURGERY---------------------

Circulating Nurse - Christine Salimbagat: First, I will

check all the equipment for proper functioning such as
cautery machine, suction machine, OR light and OR table. I
make sure the theater is clean, no unnecessary debris in the
operating room. I am arranging the furniture according to
use.

Here, I am placing a clean sheet, arm board (arm strap) and

a pillow on the OR table, as well as providing a clean kick
bucket and pail at the side of the operating table.
Collecting necessary stock and equipment.

I am going to turn on the aircon unit now.

Scrub Nurse - Neham Usman: I will now ensure that the
circulating nurse has checked the equipment as well as
ensure that the theater has been cleaned before the trolley
is set.

Scrub Nurse - Neham Usman: Circulating Nurse Salimbagat,

have you checked all the equipment?

Circulating Nurse - Christine Salimbagat: Yup! Everything

has been checked.

Scrub Nurse - Neham Usman: Now, I will prepare the

instruments and equipment needed in the operation.

Circulating Nurse - Christine Salimbagat: Meanwhile, I will

perform surgical hand antisepsis to remove debris and
transient microorganisms. As well as perform my gowning and
closed gloving taking note of the sterile technique.

Scrub nurse - Neham Usman: Yes. Thank you, Circulating

Nurse Salimbagat. For now, I’ll perform scrubbing, gowning,
gloving using sterile technique.

Circulating Nurse - Christine Salimbagat: Scrub Nurse

Usman,let me help you with setting up the theater

Scrub nurse - Neham Usman: Thank you, Ms. Salimbagat.I will

receive sterile equipment via circulating nurse Salimbagat
still using sterile technique.

Scrub Nurse - Neham Usman: Now that the surgeon is here, I

will now perform assisted gowning and gloving to the surgeon
as soon as he/she enters the operation suite.

I will now assemble the drapes according to use. I’ll start

with a towel, towel clips, draw sheets and then a lap sheet.
Then, assist in draping the patient aseptically according to
routine procedures.
I will place the blade on the knife handle using a needle
holder, assemble suction tip and suction tube. Then, I’ll
bring the mayo stand and back table near the draped patient
after draping is complete.

Now, I will secure the suction tube and cautery cord with
towel clips or allis.

Then, I will now start preparing the suture and needles

according to use.

-------------------DURING THE SURGERY----------------------

Circulating Nurse - Christine Salimbagat:I will be turning

on the OR light.

Scrub Nurse - Neham Usman: During the surgery, we must

maintain sterility throughout the procedure and adhere to
the policy regarding sponge, instruments count and surgical
needles.

Circulating Nurse - Christine Salimbagat: This time, I will

be assisting the anesthesiologist in positioning the patient
and assist the patient in assuming the position for
anesthesia.

Scrub Nurse - Neham Usman: As the scrub nurse,I am

responsible for the arrangement of the instruments on the
mayo table and on the back table.

Circulating Nurse - Christine Salimbagat:And of course, I am

anticipating the anesthesiologist’s needs, if spinal
anesthesia is contemplated, I need to place the patient in a
quasi fetal position and provide pillow, and perform lumbar
preparation aseptically.

After the patient is anesthetized, as a circulating nurse, I

will be repositioning the patient per anesthesiologist’s
instruction, attache anesthesia screen and place the
patient’s arm on the arm boards, apply restraints on the
patient, expose the area for skin preparation, catheterize
the patient as indicated by the anesthesiologist and perform
skin preparation.

*Before the incision begins*

Circulating Nurse - Christine Salimbagat: As a circulating

nurse, I need to ensure to connect diatherapy, suction,
etc., position kick buckets on the operating side on a
safest position, replenish and record sponge/ sutures,
ensure the theater door remain closed and patient’ s dignity
is upheld and watch out for any break in aseptic technique

Scrub Nurse - Neham Usman: I will now provide 2 sponges on

the operative site prior to incision. I’ll pass the 1st
knife for the skin to the surgeon with a blade facing
downward and a hemostat to the assistant surgeon. I will
then hand the retractor to the assistant surgeon.

As a scrub nurse, I must watch the procedure and anticipate

the surgeon’s needs. Watch out for hand signals to ask for
instruments and keep instruments as clean as possible by
wiping instruments with moist sponge.Also, it’s also my duty
to always remove charred tissue from the cautery tip.

If I need any additional instruments, I must notify the

circulating nurse as clearly as possible.

Circulating Nurse - Christine Salimbagat:I will remain in

theater throughout operation. Don’t worry, I will be
focusing the OR light every now and then

Scrub Nurse - Neham Usman: I am keeping 2 sponges on the

field and starting to save and care for tissue specimens
according to the hospital policy.

And oh, before I forgot. I also adhere and maintain sterile

technique and watch for any breaks to avoid contamination.
 *Preparation before closing of a cavity*

counting, this is done before a closure of a body cavity.
