AFFIDAVIT OF LOSS
I, ALMA MINA, of legal age, Filipino, married and a resident of Brgy.
Bagting, Gabaldon,Nueva Ecija after having been duly sworn to in accordance
with law, hereby depose and state that:

1. I am the safe-keeper of Transfer Certificate of Title No. XXXXXXX

which was entrusted to my custody by my sister XXXXXXXX;

2. My sister XXXXXXX is the purchaser of a parcel of land covered by

Transfer Certificate of Title No. XXXXXX with an area of Seven
Hundred Fifteen (715) square meters located at Brgy. Bagting,
Gabaldon, Nueva Ecija. A copy of the Deed of Sale between the
registered owner and XXXXXXX is hereto attached as Annex “A”;

3. Sometime on June, 2017 after the execution of the abovementioned

Deed of Sale, my sister entrusted to me the possession of Transfer
Certificate of Title No. XXXXXXX for the purpose of safekeeping
since her work obliges her to be frequently outside the Province of
Nueva Ecija;

4. I kept the owner’s copy of the XXXXXXX with my important files

kept inside a cabinet in my room;

5. Sometime on June 14, 2020, my sister asked if she can see the
Owner’s Copy of XXXXXXXX because she already gathered enough
funds to have it transferred in her name. Unfortunately, I discovered
that XXXXXXXX was not among my important files and has gone
missing;

6. That said certificate of title is nowhere to be found and despite

exerting diligent efforts to locate it, everything proved futile;

7. The property covered by Transfer Certificate of Title No. XXXXXXX

has not been mortgaged, pledged nor delivered to any person,
private entity or banking/ financial institution for any credit or
obligation whatsoever;

8. That in order to protect the interest of my sister who is the

purchaser of the property covered by XXXXXXXXX, I have executed
this affidavit for the purpose of registration with the Register of
Deeds of Nueva Ecija and in order to file a petition for issuance of
second owner’s duplicate copy of the said title in court;

9. That I am executing this affidavit to attest to all the above facts and
to request the Honorable Register of Deeds of Nueva Ecija to
annotate at the back of the original copy of Transfer Certificate of
Title No. XXXXXXXX the fact of loss of the duplicate owner’s copy
of the said certificate of title.
