the License to Operate a Dialysis Clinic of The Riverside Medical Center, Inc. located at SM City
(South Wing), Luzuriaga St., Brgy 12, Bacolod City 6100, Negros Occidental, for the reason that we will
be adding stations, from 9 stations to 12 stations.

In line with this, may I request your good office for an inspection (if possible) as soon as possible that we
may operate at the earliest possible time?
