I , Father/Mother/Guardian of studying in

Class give my CONSENT for my Ward to attend the Finalization of IB May

2021 Final exam components - EE, TOK and IA between 10th and 26th February 2021

A negative PCR COVID test administered within 72 hours of the first date my child will be
entering campus will be required for entry onto campus. By signing this form I ensure all safety
measures at home and send my ward only if he / she is healthy and fit in any following campus
visits within a 14 day window. I will ensure that my ward strictly follows the set protocol for
safety. I will ensure that he/she wears the mask and follows the instructions given by the school.

I understand that the safety and security of my ward is the responsibility of the school only till
he/she is on campus and not before and after.

My child will be dropped and picked up by
