We are pleased to inform you that your application as a MASSEUSE for our massage
parlor has been accepted. We would like to offer you a probationary employment for six (6)
months commencing on [date] up to [date], under the following terms and conditions:

1. As a probationary employee you are expected:

a. To exhibit commitment and passion for work;

b. To be honest and have integrity as a member of our parlor;
c. To fully cooperate and maintain a healthy relationship with management and your co-
workers;
d. To promptly report for work at exactly [time] until [time] Should you be required to serve
the last service call, you are expected to finish your service until its completion but not
beyond [time] which is the closing time of the branch;
e. You are expected to report for work six days in a week, with one rest day as may be
assigned by the Management. The rest day may be transferred to other days of the week
in case your service may be needed;

2. As a masseuse, your work shall be compensated based on the minimum wage.

3. The company allows masseuse like you to receive tips from customers. Tips shall not
form part and will not be included in your reported compensation;

4. In compliance with the law, the company shall deduct from your compensation all
necessary contributions to the SSS, Pag-Ibig, Philhealth, and taxes, for which the
necessary counterpart contributions from the company shall likewise be made and
remitted to the proper government agencies concerned;

5. You shall not use or disclose to any other persons or entity, for the duration of this contract
and even thereafter, any confidential information which you may obtain access to. In
order to ensure the parlor’s secrets, you are prohibited from being employed with any other
related business within one (1) year from resignation or separation from the parlor. Any
violation of this AGREEMENT shall carry with it a liquidated damage equivalent to the
masseuse total compensation for one year;

6. As masseuse, you are required to have your full attention to your employment and be loyal
to the cause and advancement of the parlor. As such you are not allowed to be employed
with other companies engaged in any activity that may compete directly and indirectly with
the parlor.

7. During this probationary period of your employment, your performance shall be reviewed
and evaluated in accordance with this contract and the parlor’s standards in order to
determine your fitness and qualifications. It is a must that during the entire period of your
employment, you abide by all existing parlor’s policies, written or unwritten as well as its
rules and regulations.
8. The parlor’s reserves the right to terminate your services at any time within this
probationary period in the event that your performance is deemed unsatisfactory or for any
legal reasons whatsoever such as alcohol or drugs abuse;

Should you be agreeable to the above rules and regulations, please sign below to signify
