This Judicial Affidavit of JESUS LEVY T. DACERA, the Police

Officer who conducted the rape investigation, is executed to serve as a
testimony to the instant case.

PRELIMINARY STATEMENT

The lawyer conducting or supervising this examination is ATTY.

ALEXNDRA DAMAYO. At Consolacion Police Station, Brgy. Poblacion
Oriental, Old Municipal Hall, M. Pepito St, Consolacion, 6001 Cebu.

I am answering his questioned, fully conscious that I do so under oath

and may face criminal liability for false testimony and perjury.

This Judicial affidavit is prepared in English and is offered to the

prove the following:

a. That I am the Police Officer who was the Investigator on Case of

Intentional Abortion
