Thank you for allowing me to administer the ASQ-3 assessment to your daughter,
Lakelyn. For this meeting we will be discussing goals for Lakelyn’s development as well as
strengths and any improvement that can be taken. The main purpose of this assessment is to
provide reliable, accurate developmental and social-emotional screening. To conduct this
assessment, I will have Lakelyn participate in a number of developmental tasks to see how well
she performs them at her age. Once I have marked how Lakelyn does on these tasks I will be
scoring each developmental area and reporting to you where she stands developmentally.
Lakelyn scored a 55 out of 60 in the fine motor skills area. She did well at using her
hands to navigate toys in various ways including, picking up crumbs with her thumb and index
finger, picking up a piece of string, and picking up a small toy with only one hand. She scored a
50 out of 60 in the communication area. She did well at saying repeated phrases like dada, gaga,
and baba. She also did well at copying sounds she hears and playing games such as peek-a-boo
when asked to. Lakelyn scored a 45 out of 60 in the personal-social area. She did well at drinking
from a cup with and without assistance, feeding herself a snack such as a cracker, and pushing
her arms through when being dressed.
Lakelyn scored a 40 out of 60 in the Gross motor area. She did well at standing and
holding her balance with assistance, sitting up straight on the floor for a long period of time, and
standing while holding onto furniture. Because she scored a 40 out of 60 there are some areas of
concern and those areas mainly include being able to pull herself up to stand, bend down and
pick up a toy and then return to standing position, and walking along side furniture. Lakelyn
scored a 40 out of 60 in the problem-solving area. She did well at passing toys from one hand to
another, picking up a toy in each hand at once, and clapping the toys together when there is one
in each hand. Because Lakelyn scored a 40 out of 60 there are some areas of concern and those
are mainly holding a toy and hitting it against another toy that’s on the table, trying to get a
crumb inside a bottle, and finding a toy that has been put underneath a cloth.
Due to these areas of concern I have recommended some developmentally appropriate
activities to assist and support Lakelyn’s growth. The first one is for gross motor and it includes
pulling up and standing while the other activity is for problem-solving and includes peek-a-boo
with toys and exploration play. These activities are described in better detail on the attached
document labeled: Lakelyn’s Learning Activities.
Overall Lakelyn is doing well developmentally and is showing no serious signs of
concern. After working on the activities that were recommended I would like to follow up and
re-administer the ASQ-3 in 3 months’ time. If there are any further questions that may arise,

Lakelyn’s learning activities

Gross Motor: Pull up and stand. Grab a few different sized carboard boxes or laundry baskets.
Flip the boxes/baskets over so that they are upside down on the floor. Encourage Lakelyn to
stand by putting some of her favorite toys on top of the boxes/baskets. Help her to stand at
first and then slowly encourage her to try it on her own. Eventually she will be able to do this on
her own as well as grab the toy on top.

Problem-Solving: Peek-a-boo toy addition. Play peekaboo with some of Lakelyn’s favorite toys.
Then, once she retrieves the toy after playing peek-a-boo encourage her to explore the toy by
modeling different ways to play with the toy. While she plays with the toy talk to her and
explain to her what she is doing.
