Kindergarten Registration for the
2021-2022 School Year

Kindergarten Registration Dates:


Knickerbocker Elementary – May 10 & May 11, 2021
North Elementary – May 10, May 11 & May 13, 2021
Ohio Elementary – May 17 & 18, 2021
Sherman Elementary – May 17 & 18, 2021
Starbuck Elementary – May 13 & 14, 2021

Please call your child’s elementary school to make an appointment for Kindergarten Registration.
If you are unsure of what elementary school your child will attend, you can call our
Central Registration Office (315)661-8338 for assistance.

Immunization Requirements for Kindergarten:


Please be aware that all students are required to provide proof of immunization under Public Health Law 2164.
For incoming Kindergarten students, the required immunization are as follows:
Vaccines Kindergarten & Grades 1, 2, 3, 4 and 5
Diphtheria and Tetanus toxoid-containing vaccine 5 doses OR 4 doses if the 4th dose was received at 4
years or older OR 3 doses if 7 years or older and the
and Pertussis vaccine (DTaP/DTP/Tdap/Td)2 series was started at 1 year or older
4 doses OR 3 doses if the 3rd dose was received at 4
Polio vaccine (IPV/OPV)4 years or older

Measles, Mumps and Rubella vaccine (MMR)5 2 doses

Hepatitis B vaccine6 3 doses

Varicella (Chickenpox) vaccine7 2 doses


Tetanus and Diphtheria toxoid-containing vaccine and
Not applicable
Pertussis vaccine booster (Tdap)3

Meningococcal conjugate vaccine (MenACWY)8 Not applicable

Haemophilus influenzae type b conjugate vaccine (Hib)9 Not applicable

Pneumococcal Conjugate vaccine (PCV)10 Not applicable
