Flat Business Line (for regular voice, visa, check verification, etc.) Please remember you must program your
equipment to dial 9. We will provide a standard modular jack in the booth space noted. Please bring a line cord
long enough to reach across your booth. All service will be restricted from long distance dialing unless noted here.

The above fees cover service beginning the Tuesday before the Fair through the last night of Fair (Sunday). If you
need service earlier, there will be an additional $45.00/month. We will make every effort to insure that you have service
in your booth by the first day of the Fair or earlier but due to the unusual dynamics of the Fair and unanticipated repairs
we cannot guarantee service. There may also be service outages and repairs that will take place during Fair. We do not
provide refunds unless we cannot provide any service in the area requested.

Please complete the section below only if you are ordering service directly through Qwest.

I have ordered a business line or DSL service from Qwest. The Qwest provided telephone or ID number is
_________________________________ and this will be delivered to the Fair on __________________. Please
schedule your service to be delivered to the Fair no later than August 25th to help insure that it is installed in your booth
by the first day of Fair. Please make sure any equipment ordered (modem, etc) is delivered to your own business address.

ISDN or Special Services ordered through Qwest – Please make arrangements for Qwest installers to extend
your service to your booth. Fair installers will assign cable pairs to Qwest. Fair installers will not provide any
labor or materials.
