These assemblies are missing shields at tube 1(counting top to bottom)at the rear
bend. There are existing pad welds at this location, recommend installing shields to
prevent any further erosion.

Grind out and re-weld the crack in the membrane weld between wall tubes 45 and 46.
Approximately 10 of the center assemblies that involve the offset tubes for access to
the rear wall in this area are dropped down approximately 1'. The assemblies need to
be aligned and reset into the support clips to prevent additional stress, condensate
lay-up, and to allow inspection of the tubes along the rear wall.

The rear loops of these assemblies are engaged with the rear wall tubes. Bring them
back into alignment to prevent abrasion between the rear wall and the assemblies.

These shields have become disengaged. Either rotate these shields back into place
and reattach or remove the shields and install new.

Due to the ash accumulation a 100% complete inspection was not attainable.
Appearance: The entire Convection Pass is covered with a layer of ash therefore
reducing the quality of inspections. Consideration should be made to thoroughly
clean the boiler during every shut down to improve inspections as well as
maintenance.
