(b) Get a Certified Copy of the Plan / Gat – plan in respect of my Client’s property as per your

Sir,

We are concerned for our Clients, Mr. Nelson Cyril D’sa and Mrs. Veronica Nelson D’sa, both
Husband and wife, resident, occupant and owner of a business – cum – residential premises
being “C. D’sa Paan Beedi Shop” situated at Plot No. 141 (as per the Municipal, Shop
Establishment Authority and other Statutory Authority records) located at Dr. E. Borjes road,
Parel, Mumbai – 400 012 (hereinafter referred to as the “said premises”).

Under the specific instructions of our Clients, We do hereby make this Application on their
behalf stating the requisite facts as follows:

1. That our Clients are residing and having exclusive peaceful occupation of a business – cum –
residential premises as stated hereinabove and they have been living at the same place and
carrying out the business of selling pan, beedi, etc. since last 70 years and for about 3
generations.

2. In almost all the records as maintained by the various Statutory Authorities have mentioned the
location of the said premises in Plot No. 141

3. To our Client’s surprise, on perusing one of the plan copy as enclosed herewith (€which seems to
be drawn and issued by a Statutory Authority) it seems that as per the geographical location, our
Clients’ said premises is well placed, constructed and established since last 70 years in Plot No.

139 and not in Plot No. 141 as mentioned in all the records as maintained by the various
Statutory Authorities including Municipal Authorities.

Hence, in the aforementioned circumstances, I hereby request your good self to kindly issue a
Copy of the Certified Plan / Gat Plan, as the case may be, confirming Our Clients’ geographical
position in your records. I do hhereby undertake to pay necessary Certified Copy charges /
Surveyors fees as applicable.

For your information, the Geographical position is:

South West corner at Crossroads of Parmar Guruji Marg and Dr. E Borjes Marg.

The same location was formerly South West corner at Crossroads of Chamar Baug Road (now
Parmar Guruji Marg) and Hospital Avenue Road (now Dr. E Borjes Marg).

Also, it is diagonally opposite to South – West corner of K. E. M. Hospital.
