OUT PATIENT DEPARTMENT

 The main outpatient pharmacy entertains prescriptions from the main outpatient clinic from
various specialists out patient’s clinics.

 Prescription drug services are a major component of outpatient services provided to eligible
patients of the Department.

 These services include direct and indirect patient medication counseling, drug and supply
dispensing services, and clinical pharmacist activities as a component of the interdisciplinary
health care team.

 All outpatient services are to be provided in a safe, appropriate, timely, and cost effective manner.
WARD SUPPLY

Ward pharmacy is the practice of pharmacy as part of a multidisciplinary healthcare team directed at
achieving the quality use of medicines.

Supplies medicine to patients in wards and specialized units and clinics.

Medicines are supplied via unit of dose, unit of use, impress stock and e pharmacy.

In the unit of use system, medicines prescribed are prepared by pharmacy and kept in the individual
drawers of the medication trolley placed in the wards.

Identification and reduction in risks associated with medicines use.

Screening of unit of use and discharge prescriptions from the ward.

Ensuring adequate drug distribution and proper storage.

SATELITE PHARMACY

Provides scientific and evidence-based literature for clinical pharmaceutical services and responds to
any query from hospital staff as well as the general public to support rational, patient and disease-
specific, cost effective drug therapy.

Supplies medicine to patients in surgical and medical wards only.

Medicines are supplied via unit of dose, unit of use, impress stock and e pharmacy.

MANUFACTURING

This unit is entrusted with the preparation, compounding and supply of both sterile and non-sterile
preparations that are not available commercially.

Medicines are also prepacked into convenient dispensing unit’s thus reducing dispensing time as well
as eliminating medication errors.

All preparations are produced under strict quality control practices.

TOTAL PARENTAREL NUTRITION (TPN)


Sterile parental nutrition preparations are made available to patients who are unable to feed orally or
enter ally.

Recommendations of parental nutrition regimens are given according to the patient's condition and the
sterility of products are ensured through routine sterility testing.

Other preparations such as extemporaneous eye drop, nasal and ear drop, IV admixture and others are
also prepared upon request.

THERAPEUTIC DRUG MONITORING (TDM)

Therapeutic Drug Monitoring Service aims to provide individualized dosage adjustments through the
measurement of serum drug levels and the application of clinical pharmacokinetics.

This enables patient-specific drug dosage regimens to be designed especially for drugs with a narrow
therapeutic window.

CYTOTOXIC DRUG RECONSTITUTION (CDR)

Cytotoxic drugs are prepared in a clean room aseptically to provide protection to the products, the
personnel and as well as the environment.

Cytotoxic waste management is implemented in accordance with the standard guidelines for
environment safety.

Reconstituting the cytotoxic drugs.

Cytotoxic drugs are well known for causing side effects such as nausea, vomiting, alopecia, mucositis,
anemia and neutropenia.

STORE MANAGEMENT

The Pharmacy Department has implemented 5 S and achieved the standard requirements of quality
environment practices certified by Malaysia Productivity Corporation (MPC) on March 2008.

CONTINUOUS PHARMACY EDUCATION (CPE)

Attended CPE twice a week to get some knowledge about new drugs.
