Major Responsibilities
Performed psychosocial assessments, and provided treatment, planning and service
delivery for long term care and rehabilitation patients, including those in hos
pice, home and palliative care. Consulted and coordinated with multidisciplinar
y team of medical professionals. Conducted family conferences. Provided counse
ling, education, information, and referrals regarding chronic disease and termin
al illness, adaptation to dementia, injury and disability. Arranged alternative
housing, transportation, and other necessary resources. Documentation of patie
nt records, including electronic charting to meet regulatory and quality assuran
ce requirements. Created and oversaw Resident Council, including attendance at
city and statewide council meetings with colleagues and leading consumers of nur
sing home/rehabilitation services.
Major Accomplishments
Responsible for all long term care patients for Group Health Cooperative (a larg
e hospital cooperative). Established first Resident Council and edited their fa
cility newsletter. Initiated first long term care exercise program focusing on
wellness in the Seattle metro area. This program became the model for future el
der fitness programs across the country.

Major Responsibilities
Complete individual and family care consultation and planning services. Assessm

ds, including care for incapacitated individuals. Provide basic financial consu
ltation including wills, trusts, beneficiary designation and property ownership.
