Objective:
Provide physician practices and their clinic with a day-to-day operational evalu
ation.
This is accomplished by:
Conducting a comprehensive evaluation and assessment of clinic operations, incl
uding physicians, managers, staff, patients and their families
Working with Physician, patients and their families, clinic staff and managemen
t in developing strategic operational direction e.g. develop and implementation
of the Patient Centered Primary Care Home Model of Care
Identifying what is operationally working well and what areas need improvement
Assist in staff development and plans for continuous improvement
