Clinical Functions:

o Supervise technical staff; ensure a safe and orderly workflow and rearrange th
e schedules to cover for lunches, breaks and call outs.
o Evaluate antibiotics and H2 antagonist orders to ensure appropriateness to pat
ients' pharmacokinetics parameters and therapy, encouraging IV to PO conversion
where applicable. Do Pharmacokinetics calculations on all first dose antibiotics
, with subsequent follow up based on levels.
o Review profiles with narrow therapeutic index medications as well as document
SCr, BUN, weights and other pertinent data.
o Provide drug information to Medical staff, Nurses and the community. Advised a
nd encouraged the use of therapeutic equivalent substitutions for non-formulary
drugs and restricted antibiotics prescribed. Intervene on these orders and docum
ented as necessary.
o Respond to all codes and assist with drugs as needed.
o Initiate Coumadin protocols, write and evaluate labs 48 hours post therapy ini
tiation.
o Designed Pharmaceutical Care plans aimed at targeting potential drug related p
roblems for the home care patients. Evaluated therapy for desired outcomes. Part
icipate in multidisciplinary care plans, rounds and discharge planning.
o Reconcile nursing MARs with pharmacy profiles in an attempt to decrease medica
tion administration errors documenting all Errors and ADRs.
